<p>JobID: 20225984</p> <p>Category: Center</p> <p>JobSchedule:</p> <p>Posted Date: 2026-02-12T04:24:25+00:00</p> <p>JobShift: 1st</p> <p>Location Type: Skilled</p> <p>Great new Career Opportunity!</p> <p>Now Hiring - Advanced Practice Nurse - Bridge Care Bergen County NJ</p> <p>Salary Range $125,000 to $135,000 a year</p> <p>Advanced Practice Nurse will be responsible for, but are not limited to:</p> <ul> <li>Conduct visits to patients in accordance with local, state and federal regulations. </li><li>Communicate visit findings to designated members of the interdisciplinary team. </li><li>Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment). </li><li>Consult with other members of interdisciplinary team to contribute to the patient's plan of care. </li><li>Prepare and maintain accurate patient records, charts and documents. </li><li>Assist in development of clinical practice guidelines/standards in support of quality care as needed. </li><li>Ensure collaboration with patients, caregivers, medical directors and members of IDT. </li></ul> <p>Education:</p> <p>Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.</p> <p>Professional Experience:</p> <ul> <li>Must have a Nurse Practitioner's license in the state of operations </li><li>Must have 1+ years' experience working within Family Practice, General Practice, Geriatric care or related field, </li><li>Must have a valid driver's license and a reliable source of transportation </li><li>Must be willing to travel </li><li>Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required. </li><li>Commitment to ongoing professional development and clinical excellence </li></ul>
