<p>About this position as a Social Work and Spiritual Care Clinical Specialist</p> <p>The Social Work and Spiritual Care Clinical Practice Specialist is responsible for the oversight of the assessment, planning, implementation, and evaluation of care provided by social work and chaplain staff in clinical operations in collaboration with their direct clinical leader. This role is directly responsible for the professional competency oversight and professional development of social work and chaplain staff, students, and hospice interns. This role serves as a member of the Clinical Operations Team to facilitate the best practices and address concerns/problems associated with the delivery of clinical services.</p> <p>This is a full time 40 hours a week position from 8:00am-4:30pm</p> <p>What will you do as a Social Work and Spiritual Care Clinical Specialist?</p> <ul> <li>Represents HopeHealth Hospice in public forums, professional forums and partnerships which include exposure across multiple platforms/venues. </li><li>Represents psychosocial disciplines and end of life care, as a Subject Matter Expert. </li><li>Develops the implementation of practice theory into practice application. </li><li>Supervises and/or oversees provision of state regulated supervision, and/or coordinates other supervision resources, of master's prepared social work staff for advancing to independent or clinical licensure and/or to meet the state requirements for provision of required supervision hours monthly. </li><li>Performs gap analysis for organizational needs in Social Work and Chaplain practice and plans mitigation strategies in collaboration with Clinical Operations leadership to align with strategic initiatives. </li><li>Orients and onboards new Social Workers and develops orientation and onboarding plan for new Chaplains. Assigns preceptor for new Chaplains and monitors onboarding process. </li><li>Carries a patient caseload and performs patient assessments and follow up visits. Caseload size will vary depending on needs of the organization. </li><li>Participates in Social Worker on call rotation. </li><li>Performs chart audits of discipline specific documentation and coaching staff on best practices of documentation based on audit outcomes. Coordinates with clinical management when remediation is needed. </li><li>Performs competency assessments and contributes to annual performance evaluations, for all Social Workers, in collaboration with clinical management which may also include remediation or performance improvement plans for staff. </li><li>Serves as a mentor to new and current employees in a capacity which may require direct patient care. Through mentorship, mitigates practice gaps; moves clinical employees from the position of novice or intermediate clinician to a skilled clinician in end-of-life care with an expectation that all clinical staff practice at the highest level of competency for their discipline and according to the standards of practice. </li><li>Develops preceptors to ensure learning outcomes for new psychosocial staff or students is achieved. </li><li>Supervises social work students to meet their on-site learning objectives in collaboration with their institutions for social work intern placement requirements when applicable. Coordinates with Clinical teams for students to engage in patient care experiences. </li><li>Collaborates with appropriate departments (such as IT) for ongoing practice enhancements in the use of the Electronic Medical Record. Trains staff on the use of the EMR and documentation standards. Performs audits and quality checks in support of performance improvement related to EMR use. </li><li>Functions as a resource to clinical managers and staff, supporting specified teaching needs in the area of safe and effective delivery of care and continues to develop protocols for best practices in areas of expertise. </li><li>Provide Just-in-Time Training when remediation needs are identified, or performance improvement activities are required. </li><li>Assists with performance competencies (discipline-specific), skills checklists, and other daily team operations as assigned. </li><li>Functions as a resource to clinical managers and staff, supporting specified teaching needs in safe and effective delivery of care and continues to develop protocols for best practices in areas of expertise. </li><li>Attend Interdisciplinary Team (IDT) meetings as needed and offer coaching/mentoring for core discipline chaplain and social worker participation. Provide education to IDT members on the nature and scope of chaplain and social worker psychosocial care. Coach skills in the other disciplines providing social work and spiritual care. </li><li>Attends/completes mandatory in-services and/or meetings or makes arrangements with the appropriate supervisor for satisfactory compliance. </li><li>Supports and participates in HopeHealth initiatives / QAPI / performance improvement activities and committees as designated by Chief Clinical Officer. </li><li>Other duties as assigned by the Chief Clinical Officer. </li></ul> <p>What are the qualifications to be a Social Work and Spiritual Care Clinical Specialist?</p> <ul> <li>Master's in social work (MSW) from an accredited educational institution with LICSW certification plus two or more years' experience in clinical practice. </li><li>Licensed to provide Social Work Supervision in the State of Rhode Island and Massachusetts required. Completed application for Rhode Island and/or Massachusetts social work licenses within 30 days of hire. </li><li>Minimum of two years of experience providing direct, state-authorized supervision to Social Workers seeking independent or advanced licensure, required. Five or more years preferred. </li><li>Prior hospice and/or palliative care healthcare experience required, will consider other end-of-life healthcare settings. Interdisciplinary teamwork experience required. </li><li>Position will be point clinical specialist as described here and treating MSW and percentage of time spent will fluctuate as necessary to meet the needs of the organization. </li><li>Possess a professional maturity that consistently sets the tone in carrying out the responsibilities of the positi </li><li></li><li>on, exercising the highest degrees of integrity, and confidentiality. </li><li>Demonstrated knowledge of and skill in oral communication, written communication, customer service, patient and family experience, conflict resolution, problem solving, quality management, and systems thinking. </li><li>Skilled in motivating, developing and directing people as they work to enlist cooperation of many people to perform/achieve a common goal. </li><li>Skilled in identifying many aspects of complex problems, able to review related information and then develop and evaluate options and implement solutions. </li><li>Advanced proficiency in the Electronic Medical Record required within 90 days of hire. </li><li>Demonstrated knowledge and application of Accreditation standards, hospice regulations, OSHA, CDC regulatory standards and applicable state and federal regulations. </li><li>Ability to exercise initiative and independent judgment. </li><li>Demonstrates the ability to work efficiently and effectively under pressure, with attention to details and deadlines and to simultaneously handle multiple projects. </li><li>Demonstrated organizational and time management skills. </li><li>Demonstrates the willingness and ability to function as an integral part of a team, including working evenings and weekends, if needed, to accommodate the needs of the organization and the training needs of all shifts. </li><li>Advanced Proficiency in Microsoft Office products: Outlook, Word, Excel, and Power Point.
