<p>Department</p> <p>Integrated Case Management</p> <p>Job Summary</p> <p>This leader has operational accountability and oversight, and manages the daily activities of the department or responsible area and supports best practice standards for department functions and the Case Management Program. In this role, the manager sets the tone and models positive leadership behavior, while ensuring that team work tasks, projects, and responsibilities are completed successfully in support of departmental and organizational goals. The Manager adheres to all Federal, State and regulatory statues and accreditation requirements. The Manager supports the Director in developing, planning and implementing appropriate changes in keeping with case management best practice standards and organizational goals.</p> <p>Required Qualifications</p> <p>Education Requirement:</p> <ul> <li>Require graduate of an accredited school of Nursing or Social Work. Social Worker: - Require a Master of Science in Social Work (MSW) degree. </li><li>Registered Nurse: - Require Bachelor of Science in Nursing (BSN) degree; Master of Science in Nursing (MSN) degree is preferred. </li></ul> <p>Licensure Requirement:</p> <ul> <li>For Registered Nurse: - Require active State of Florida Registered Nurse licensure. </li><li>For Social Worker: - Require active State of Florida Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC). </li></ul> <p>Certification Requirement:</p> <ul> <li>For Registered Nurse: - Require Certified Case Manager (CCM) or Certified Advanced Case Manager (C-ACM) certification. </li><li>For Social Work: - Require Certified Social Work Case Manager (C-SWCM) or Certified Advanced Social Work Case Manager (C-ASWCM) certification. </li><li>Require three (3) to five (5) years of case management or other healthcare related experience with at least one (1) year in a supervisory/managerial role, preferably within an acute care hospital setting. </li></ul>
