<p>SUMMARY Under the direct supervision of the CCSO Program Manager, the Care Coordination Supervisor performs general functions, including direct oversight of the day-to-day operations with assigned CCSW Care Coordinators and CCSI Care Coordinators to ensure appropriate and timely delivery of services. The Care Coordination Supervisor assists the CCSO Program Manager with ensuring the Care Coordination Support program is in compliance with federal and state requirements. The Care Coordination Supervisor follows all agency policies and procedures, including those established by the funder, as well as successfully attaining all required funder benchmarks. The Care Coordination Supervisor will be responsible for providing direction, support and guidance to Care Coordinators. The Care Coordination Supervisor delivers excellent quality care, positive customer service, the maintenance of an efficient and safe environment of care. Demonstrates superior customer service skills and has the ability to function in a multi-tasking, multiple priorities environment, while maintaining accuracy and attention to detail.</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES</p> <ul> <li>• Directly oversee CCSW Care Coordinators and CCSI Care Coordinators. </li><li>• Develop goals and expectations based on program requirements and set the direction for Care Coordination Staff. </li><li>• Maintain patient confidentiality at all times per the policies and procedures. </li><li>• Maintain ethical behavior at all time per the policies and procedures. </li><li>• Responds to internal and external customer needs in a positive manner, utilizing principles of the AIDET model; exceeding their needs and expectations and providing the highest quality service to ensure the best possible outcomes. </li><li>• Master the electronic health record system, Athena, to appropriately document patient data and information, in addition to managing databases with outcomes data. </li><li>• Complete all required reports and documentation as required by the funders. </li><li>• Collaborate with staff within the Care Coordination department, as well as, other agency divisions and community partners to enhance coordination of care. </li><li>• Monitor and analyze the collection of required benchmark data. </li><li>• Supervise and serve as a consultant for Care Coordinato and assist them in the development and enhancement of skills by providing individual supervision, performance reviews and team meetings. </li><li>• Ensure the agency requirements and expectations are communicated and enforced. </li><li>• Participate in multi-disciplinary staffing to ensure the coordination and appropriate delivery of patient care. </li><li>• Responsible for communicating and ensuring documentation of any issues requiring the completion of Incident Reports. </li><li>• Attend all trainings and meetings as required by the program. </li><li>• Complete and submit all agency and program paperwork in the appropriate period. </li><li>• Attend all agency and program meetings as designated by supervisor </li><li>• Complete all Core Curriculum training(s) required for this position. </li><li>• Perform all other duties as assigned. </li></ul> <p>COMPENSATION AND BENEFITS</p> <ul> <li>Competitive Pay- $25.40-$26.44 - hourly rate </li><li>Health, Dental, and Vision Insurance </li><li>Disability & Life Insurance </li><li>Paid Vacation, Sick Days, and Holidays </li><li>Retirement Plan </li><li>Employee Assistance Program (EAP) </li><li>Employee Discounts on services like cell phones, restaurants, and more </li></ul> <p>KEY COMPETENCIES:</p> <ul> <li>• Dependable and reliable, giving extra effort when necessary to accomplish Aunt Martha's Health & Wellness goals. </li><li>• Takes initiative to proactively recognize problems/opportunities and develop appropriate strategies to deal with them. </li><li>• Ability to manage multiple and competing demands and establish priorities. Communicates decisions promptly and clearly. </li><li>• Excellent organizational skills and attention to detail, deadlines, and program compliance. </li></ul> <p>QUALIFICATIONS</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.</p> <p>EDUCATION and/or EXPERIENCE</p> <ul> <li>• Master's in Social Work or Counseling with at least 2 years' experience in mental health services and 2 years of supervising experience </li><li>• Experience in working with Microsoft Office programs is essential; adept at data entry </li><li>• Knowledgeable about community resources </li></ul>
