<p>Responsibilities:</p> <ul> <li>Responsible for providing comprehensive primary care, coordination of 24-hour care, medical coverage, and directly provides services to ArchCare participants where they live, in congregate housing, Adult homes, and home visits. </li><li>Provides face-to-face comprehensive History and Physical assessments on new referrals, and on existing patients. Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants with a full comprehensive face to face exam including pts institutionalized. </li><li>Provides evaluation and treatment of participants with acute illnesses, and works with other staff to ensure patient is safe and healthy. </li><li>Attends IDT's and morning report within your area. </li><li>Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan into the plan of care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning. </li><li>Documents changes in the participant's condition and details care provided by completing all required documentation. </li><li>Collaborates with nurse, nurse practitioner and other physicians to ensure comprehensive and continuous care. </li><li>Assists in monitoring the medical care of enrolled members who are hospitalized in out-of-network and out-of-service area facilities and assists in their transfer to in-network facilities when medically appropriate. </li><li>Attends and participates in scheduled staff meetings and participant care meetings as requested. </li><li>Participates in meetings for PCP's such as QI, IC and UM, and updates on CMS rulings. </li><li>Speaking with consultants/Specialists regarding patient care </li><li>Responsible to keeping our EMR inputting progress notes and reviewing consultation recommendations, up to date on a daily basis. </li><li>Participate on call, on call is Friday to Friday x 7 days every 8 weeks </li></ul> <p>Qualifications:</p> <ul> <li>1 year of post graduate experience in Internal or Family Medicine </li><li>Board Certified in Internal Medicine or Family Practice </li><li>Completion of residency in Internal or Family Medicine </li><li>Geriatric experience preferred </li><li>Bilingual preferred </li></ul> <p>Education:</p> <ul> <li>MD or DO Degree </li></ul>
