St. Luke's is also a Magnet hospital for nursing excellence.</p> <p>The Long-Term care Nurse Practitioner works in collaboration with the Medical Director, Case Manager, and post-acute care team members to deliver care to patients. This work includes coordinating and facilitating the delivery of care services to patients by performing the following activities to include, but not limited to: performing physical, psychosocial and spiritual assessments, detailing patient history, establishing goals of care, and evaluation of patient's status, participation in the development of a plan of care with the patient / family, attending physician and interdisciplinary team and collaborating with next level of care provider(s) in transitioning care and services.</p> <p>Responsibilities</p> <ul> <li> <p>Full time position: Monday - Friday 9AM-5PM</p> </li><li> <p>Ability to travel in between facilities</p> </li><li> <p>Position will cover several facilities with a schedule:</p> </li><li> <p>Methwick Clinic - Mon/Wed/Friday, 9am-noon with rounding after clinic</p> </li><li> <p>Cottage Grove: two ½ days per week (skilled patients)</p> </li><li> <p>Winslow House: one ½ day per week (long-term patients)</p> </li><li> <p>Linn Manor: ½ day per week (long-term patients)</p> </li><li> <p>Performs assessment process to include patient history, physical, psychosocial and spiritual assessment.
