<p>General Summary</p> <p>Provides group and individual Diabetes Education. Provides nutritional counseling to clients referred by a provider. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan with each program participant.</p> <p>Duties and Responsibilities</p> <p>Essential Functions:</p> <ul> <li>Provides group education and individual education in Diabetes Education. </li><li>Provides nutritional counseling to clients referred by a provider. </li><li>Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants. </li><li>Manages blood glucose for patients. </li><li>Manages insulin pumps and continuous glucose monitors for patients. </li><li>Collects program participant assessment data, using the ADDE7 framework, in a collaborative and ongoing manner. </li><li>Collaboratively develops an individualized follow-up plan with each program participant. </li><li>Conducts a follow-up assessment upon completion of program services, using outcome measures from the AADE7 Continuum of Outcomes Framework. </li><li>Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records. </li><li>Utilizes a team approach to provide services and collaborates and communicates with team members when needed. </li><li>Participates in peer review process to evaluate performance of other professional staff. </li></ul> <p>Common Expectations:</p> <ul> <li>In-depth knowledge about current diabetes treatment management. </li><li>Ability to develop a collaborative, therapeutic alliance with individuals. </li><li>Provides outstanding customer service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. </li><li>Enhances professional growth and development through participation in educational programs, current literature, conferences, and workshops. </li></ul> <p>Qualifications</p> <p>Minimum Education:</p> <ul> <li>Associates Degree Registered Nurse Required </li><li>Bachelors Degree Preferred </li></ul> <p>Work Experience:</p> <ul> <li>2 years Relevant experience. Required </li></ul> <p>Licenses:</p> <ul> <li>Licensed Registered Nurse Upon Hire Required or </li><li>Registered Nurse Multi State License Upon Hire Required or </li><li>Licensed Dietitian-Nutritionist Upon Hire Required and </li><li>Heartsaver CPR/AED Upon Hire Required </li><li>Certified Diabetes Care and Education Specialist within 3 years Preferred </li></ul> <p>Knowledge, Skills, and Abilities:</p> <ul> <li>Able to lead and manage groups. </li></ul> <p>WellSpan Health's vision is to reimagine healthcare through the delivery of comprehensive, equitable health and wellness solutions throughout our continuum of care.
