<p>Internal Medicine Provider</p> <p>Salary: $200,000+/Year based on experience</p> <p>LaSante Health Center is seeking an Internal Medicine Provider to join our multispecialty, community-based health center in Brooklyn. The ideal candidate will be committed to delivering comprehensive primary and preventive care to adult patients, managing chronic conditions, and promoting overall wellness in a culturally diverse community. Creole-speaking candidates are strongly preferred.</p> <p>Responsibilities:</p> <ul> <li> <p>Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management</p> </li><li> <p>Diagnose and treat acute and chronic medical conditions</p> </li><li> <p>Collaborate with multidisciplinary teams to coordinate patient-centered care</p> </li><li> <p>Counsel patients on lifestyle modification, disease prevention, and medication management</p> </li><li> <p>Maintain accurate, timely documentation in the electronic health record (EHR)</p> </li><li> <p>Participate in quality improvement initiatives and team meetings</p> </li><li> <p>Complete additional tasks as directed by supervisor</p> </li></ul> <p>Qualifications:</p> <ul> <li> <p>Medical degree (MD, DO, NP, or PA) from an accredited institution</p> </li><li> <p>Board certification or eligibility in Internal Medicine or Adult Primary Care</p> </li><li> <p>Current New York State license or eligibility for licensure</p> </li><li> <p>Experience in community health or outpatient primary care preferred</p> </li><li> <p>Excellent communication and interpersonal skills</p> </li><li> <p>Bilingual Creole/English strongly preferred</p> </li></ul>
