<p>ESSENTIAL JOB DUTIES:</p> <p>To perform this job, an individual must accomplish each essential function satisfactorily, with or without a reasonable accommodation.</p> <ul> <li>Assess, plan and provide a comprehensive plan of care that proactively manages healthcare issues and addresses barriers along the care continuum for patients in the nursing home setting </li><li>Provide medical care within the scope of the practitioner's training and consults with supervising physician when necessary and appropriate </li><li>Assess and determine appropriate medical and psychological intervention (as applicable) </li><li>Prescribe medications and services as indicated </li><li>Order appropriate laboratory studies, diagnostic procedures, and consultations </li><li>Monitor compliance with medications and evaluate patient response to plan of treatment; modify plan of treatment when necessary </li><li>Discuss and initiate Advance Care Planning for assigned patients when appropriate. </li><li>Complete necessary visits required by Medicare as well as appropriate acute visits to care for individuals. </li><li>Provide complete and correct billing information on all patient visits, consultations and procedures to office staff (as applicable) </li><li>Assist with on-call service with physician staff to provide 24/7 access to care for patients. </li><li>Participate in the development of policies and procedures relating to the provision of clinical and related services </li><li>Participate in the review of quality related data that is related to clinical practice and outcomes </li><li>Conduct informal facility staff education and teaching to improve the effectiveness and efficiency of care delivery </li><li>Perform administrative tasks and other duties as required </li></ul> <p>JOB REQUIREMENTS:</p> <ul> <li>Ensure compliance with local, state, and federal regulatory agencies related to clinical services provided by NPs or PAs </li><li>Practice in a cost-effective manner </li><li>Maintain thorough and timely documentation of services rendered in the patient's chart </li><li>Successful completion of required training </li><li>Handle multiple priorities effectively </li><li>Independent discretion/decision making </li><li>Make decisions under pressure </li><li>Reliable transportation </li><li>Current automobile insurance according to company policy </li></ul>
