<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Day Shift</p> <p>Description:</p> <p>COMMUNITY HEALTH WORKER LEAD</p> <p>SUMMARY</p> <p>A Lead Community Health Worker (Lead CHW) is a seasoned, frontline public health worker who is a trusted member of and/or has a remarkable understanding of the community served.</p> <p>Under general supervision, provides day-to-day leadership to a team of Community Health Workers, who are working to improve health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. In this role, the Lead CHW will serve as a coach and mentor to Community Health Workers and ensure compliance with program guidelines. Acts as a link between the Community Health Workers, health system teams, and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery.</p> <p>REQUIREMENTS:</p> <ul> <li> <p>High school diploma or GED. Three to five (3-5) years of community involvement experience with increasing level of responsibility. Must meet additional state/local requirements. People management experience and CHW certification preferred.</p> </li><li> <p>Must possess a comprehensive knowledge of the local community based on personal lived experience.</p> </li><li> <p>Basic Life Support (BLS) certification is preferred.</p> </li></ul> <p>HOURS: This position will be a hybrid with two days in the office (Boise), traditional day-time office hours.</p> <p>WHAT YOU WILL DO:</p> <ul> <li> <p>Provides leadership in the day-to-day functions of 9-10 CHW team members that includes new colleague training.</p> </li><li> <p>Provides manager with updates regarding CHWs' daily responsibilities, performance, and productivity, as well as internal resource needs.</p> </li><li> <p>Plays an integral role in program management, fostering growth of CHWs, ensuring quality of service delivery, and guiding progress towards intervention goals.</p> </li><li> <p>Responsible for the daily functions of a team of CHWs, including health system navigation, health education, patient engagement and follow-up, resource advocacy, and outreach activities.</p> </li><li> <p>Assists with assigning incoming referrals and monitoring volume of referrals of assigned CHWs.</p> </li><li> <p>Serves a limited caseload in a similar manner as CHWs.</p> </li><li> <p>Assists with program design, evaluation, and evolution.</p> </li><li> <p>Schedules and facilitates team huddles, meetings, and regular check-ins with each CHW to provide feedback, instruction, and recommendations for colleague development.
