<p>Community Habilitation Trainer</p> <p>Location: 1825 Atlantic Avenue, Brooklyn 11233</p> <p>Hours: Part time - 20 - 25 hours per week</p> <p>Salary Range: $16.50 -17.00/hour</p> <p>Position Summary:</p> <p>Provide Individualized services both in the community and in the Individuals home as prescribed by OPWDD Community Habilitation Plan.</p> <p>Responsibilities:</p> <ul> <li> <p>Provide hands on training to Individuals</p> </li><li> <p>Maintain Documentation and submit within established time guidelines</p> </li><li> <p>Input in development and revision of Individualized Community Habilitation Plan</p> </li><li> <p>Travel in Individuals neighborhood and community</p> </li><li> <p>Maintain Communication with families/caregivers</p> </li><li> <p>Comprehensive Travel Training</p> </li><li> <p>Encourage community participation and socialization</p> </li><li> <p>Assessment, training and assistance in developing appropriate social behaviors and community skills</p> </li><li> <p>Training and assistance in developing basic skills in safety, housekeeping, personal care, health care and money management</p> </li><li> <p>Other tasks as may be required</p> </li></ul> <p>Qualifications Required:</p> <ul> <li> <p>High School Diploma or equivalent</p> </li><li> <p>Experience or training working with Individuals with intellectual and Developmental Disabilities and/or experience and/or knowledge in the I/DD field a plus</p> </li><li> <p>Must have good interpersonal and verbal communication skills</p> </li><li> <p>Must take direction well and be receptive to feedback</p> </li><li> <p>Must be computer literate and respond well to supervision</p> </li><li> <p>This position requires fingerprinting and a criminal background check</p> </li><li> <p>Experience or training working with Individuals in a community or home setting preferred</p> </li></ul> <p>BCS is an Equal Opportunity Employer.
