We're expanding our home health services and looking for a compassionate full-time Home Health LPN to join our team in the Port Charlotte/North Port area.</p> <p>This is an exciting opportunity to serve patients in their homes with the flexibility to set your own schedule and grow with us as we continue to build our presence in the community. In this full-time role, you'll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.</p> <p>Locations: Home health visits in Port Charlotte, North Port, Arcadia, Englewood, Rotonda West, and occasionally north Punta Gorda</p> <p>Schedule: M-F 40 hours; weekend and holiday on-call rotations required.</p> <p>With more than 60 years of expertise, Empath's Home Health service line provides skilled nursing, therapy, and personal care services that help people recover and maintain independence at home.
