<p>Mountain Community Health Partnership (MCHP) is seeking a full time Care Manager. The Care Manager plays a vital role in coordination comprehensive care for patients across the continuum of health services. This position involves assessing, planning, implementing, and monitoring care plans to achieve quality outcomes. Works collaboratively with providers, patients, caregivers, and community resources to ensure care is patient focused.</p> <p>Key Responsibilities:</p> <ul> <li>Conduct comprehensive assessments addressing medical, psychosocial, behavioral, and cultural needs. </li><li>Develop and implement individualized care plans in partnership with patients and care teams. </li><li>Identify barriers to care and implement interventions to improve health outcomes. </li><li>Provide patient education on diagnoses, medications (RN), and self-management strategies. </li><li>Coordinate referrals and serve as a liaison among patients, providers, and community resources. </li><li>Monitor progress toward patient-centered SMART goals and adjust plans as needed. </li><li>Support organizational performance improvement initiatives and data analysis. </li></ul> <p>Qualifications:</p> <ul> <li>Education: Bachelor's degree from an accredited school of nursing with a license to practice in NC or Master's degree in social work (MSW) from an accredited program. </li><li>Experience: Minimum of five years' experience in an ambulatory/care management setting or minimum of five years' experience in care management, behavior health, or social work setting. Strong communication and organizational skills.
