It provides an individualized, enriched, trans-disciplinary, and integrated educational program based on clinical evidence-based research standards to facilitate whole-person development.</p> <p>Job Summary</p> <p>Job Summary</p> <p>DESCRIPTION OF POSITION: Under the direction of the Administrator/Director of Education (DOE), the Special Education-Transition Specialist provides appropriate and meaningful transition activities to post-secondary work, educational programs, and life activities. The Special Education-Transition Specialist plans, delivers, and monitors the transition services for all students with disabilities. Troubleshoots all problems related to Workability. Complete all reports, budgets, and attend required meetings. ESSENTIAL DUTIES AND RESPONSIBILITIES: Drafts written guidance for all staff who participate on IEP teams on how to evaluate transition needs and develops transition plans in compliance with federal and state legal standards Disseminates guidance to all staff who participate in IEP team meetings for students who are 16 years or older. Review each 12th-grade student's IEP to ensure that it includes a transition plan individually tailored to meet the student's needs in accordance with federal legal standards and the District's policies and procedures. Determines whether the students' IEPs include transition plans that were developed in accordance with federal legal standards and the District's policies and procedures at the students' next scheduled IEP. Participates as a member of the District Office of Transition Services team. Collects, provides data, and reports on students with disabilities receiving transition services Implements District guidelines, programs, and procedures to provide compliant transition planning and service delivery for students with disabilities. Presents and coordinates staff development programs. Utilizes technology programs to maintain accurate records and prepare reports. Plan for and assist in training staff to ensure accurate data collection.
