We will offer the vaccination at no cost via our Health Centers.</p> <p>SERVICE AREA: Housing & Justice Related Services (HJRS)</p> <p>PROGRAM: New Pathways Project (NPP) Recovery Support Services (RSS)</p> <p>JOB OVERVIEW:</p> <p>The Recovery Support Services (RSS) program is an enhancement service for the New Pathways Project (NPP) that supports individuals in recovery from substance use disorders. The RSS program provides non-clinical services that assist individuals in recovery in gaining the skills and resources needed to initiate, maintain, and sustain long-term recovery.</p> <p>The Recovery Support Outreach Case Manager provides individualized case management services to individuals in recovery. This role involves assessing client needs, developing recovery plans, connecting clients with resources, and offering ongoing support to promote long-term stability and well-being. The Case Manager serves as an advocate, ensuring clients receive appropriate services while fostering a person-centered and strengths-based approach to recovery. The Case Manager coordinates activities with members of the NPP Recovery Support Services program.</p> <p>MISSION & VISION:</p> <p>PHMC works to create and sustain healthier communities through partnerships with government, foundations, businesses, and community-based organizations. Additional duties may be assigned as needed:</p> <ul> <li>Conduct comprehensive assessments of clients' strengths, needs, and goals within the first 30 days of enrollment, with updates to be made every 6 months. </li><li>Develop and implement individualized recovery support plans. </li><li>Provide case management services, including linking clients to housing, employment, healthcare, and social services. </li><li>Collaborate with clinical providers, peer recovery support specialists, and community organizations to ensure coordinated care. </li><li>Be responsible for community outreach and external field follow-up with NPP participants (minimum 2 days in the field). </li><li>Monitor client progress and adjust support plans as needed. </li><li>Educate clients on harm reduction strategies, coping skills, and wellness practices. </li><li>Facilitate support groups, workshops, or educational sessions as needed. </li><li>Maintain accurate and timely documentation in compliance with organizational and regulatory requirements. </li><li>Advocate for clients' needs in healthcare, legal, and social service systems. </li></ul> <p>WORK HABITS:</p> <ul> <li>Demonstrates strong organizational skills and attention to detail </li><li>Maintains confidentiality and adheres to ethical standards </li><li>Works independently while also collaborating effectively with team members </li><li>Practices cultural humility and fosters an inclusive, supportive environment </li><li>Exhibits empathy, patience, and a nonjudgmental approach to client interactions </li></ul> <p>PROFESSIONAL DEVELOPMENT:</p> <ul> <li>Participates in ongoing training related to recovery, case management, trauma-informed care, and evidence-based practices </li><li>Seeks supervision and feedback to enhance professional growth </li><li>Maintains relevant certifications and licensure as required by PHMC </li></ul> <p>KNOWLEDGE, SKILLS, AND ABILITIES:</p> <ul> <li>Strong understanding of substance use disorders, mental health conditions, and recovery principles </li><li>Knowledge of local and national recovery resources, including housing, employment, and healthcare services </li><li>Excellent communication, interpersonal, and advocacy skills </li><li>Ability to de-escalate crisis situations and connect clients with appropriate interventions </li><li>Proficiency in case documentation and basic computer skills </li></ul> <p>WORK ENVIRONMENT & LOCATIONS:</p> <ul> <li>Primary Location: 2539 Germantown Ave., Philadelphia, PA 19133 </li><li>Work is primarily conducted in an office or community settings. </li><li>Some travel is required to accompany clients to appointments or provide outreach services. </li><li>May involve occasional evening or weekend hours based on client needs </li></ul> <p>PHYSICAL REQUIREMENTS:</p> <ul> <li>Ability to manage high-stress situations </li><li>Regular data entry </li><li>Ability to sit, stand, and walk for extended periods when necessary </li><li>Ability to lift up to 25 lbs. Occasionally </li><li>Ability to work in diverse environments </li></ul> <p>QUALIFICATIONS:</p> <ul> <li>Bachelor's degree in social work, psychology, counseling, or a related field required </li><li>Minimum of two years of experience in case management, behavioral health, or recovery support services </li><li>Experience working with diverse populations and knowledge of trauma-informed care principles </li><li>Preferred: Lived experience with substance use or mental health recovery and/or certification as a Certified Recovery Specialist (CRS), Certified Peer Specialist (CPS), or equivalent </li><li>Preferred: Previous experience in a harm reduction, recovery-oriented, or community-based setting </li><li>Preferred: Bilingual abilities (Spanish or other languages) </li></ul> <p>SALARY GRADE: 15</p> <p>PHMC is an Equal Opportunity and E-Verify Employer.</p>
