Our service offering includes primary medical care, behavioral health, pharmacy and dental services in an integrated model that provides a comprehensive approach to care for our patients.</p> <p>Position Overview</p> <p>The Integrated Behavioral Health Clinician (IBH Clinician), as part of the primary care treatment team, provides diagnosis, brief behavioral health intervention, and psychotherapy for patients with mental health and substance use disorders. Designated clinicians within the IBH department provide long-term psychotherapy to patients with serious mental health and substance use disorders.</p> <p>This position will float through all three clinics - Wilson Community Health Center, Harvest Family Health Center, and Freedom Hill Community Health Center.</p> <p>THIS POSITION OFFERS A 40-HOUR WORK WEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-WEEKLY PAY.</p> <p>Essential Tasks</p> <ul> <li>Educate staff via in-services on related behavioral health topics when needed. </li><li>Teach treatment enhancement techniques using non-stigmatizing language and empathy to patients, families, and staff. </li><li>Assess the clinical status of patients referred by primary care providers and outside referral sources to </li></ul> <p>develop a patient focused plan of care for substance use disorder and/or mental health treatment.</p> <ul> <li> <p>Incorporate chronic disease management into overall treatment goals.</p> </li><li> <p>Use evidence-based assessment screening tools with patients to assist with identifying concerns and</p> </li></ul> <p>outcome measurements.</p> <ul> <li>Consult with medical providers as necessary, and refer cases to specialty, case management, and other </li></ul> <p>behavioral health/substance use providers as appropriate.</p> <ul> <li>Work with primary care team to treat and manage patients with substance use disorders efficiently and effectively to improve clinical outcomes. </li></ul> <p>Experience and Education</p> <ul> <li>Experience providing care to patients with mental health and substance use disorders. </li><li>Case management experience preferred. </li><li>Graduate of an accredited university with a master's degree in social work. </li><li>Fully or provisionally licensed as a Licensed Clinical Social Worker without restrictions in the State of North Carolina. </li><li>BLS Certification. </li><li>Active North Carolina Driver License. </li></ul> <p>Schedule</p> <p>Monday, Tuesday, Wednesday, Thursday, Friday</p> <p>8-hour shift, day shift / night shift (depending on the schedule of the clinic)</p> <p>Physical Requirements</p> <ul> <li>Potential sitting or standing for long periods of time, occasional bending, stooping, lifting including of patients, reaching forward and overhead.
