Works directly with team members, patients and community members to ensure everyone over the age of 18 has ACP knowledge and a Power of Attorney for Health Care document. Provides education and support in the hospital and ambulatory setting while documenting in EPIC as necessary.</p> <p>Job Description:</p> <p>Schedule:</p> <ul> <li> <p>Full time, benefited position</p> </li><li> <p>40 hrs/week</p> </li><li> <p>Travel to various sites around the system doing ACP (advance care planning) clinics.</p> </li><li> <p>Help complete advance directives in the inpatient setting</p> </li></ul> <p>Key Accountabilities</p> <ul> <li> <p>Facilitate monthly group classes and weekly individual ACP clinics.</p> </li><li> <p>Coordinate ACP volunteers in the hospital and ambulatory settings.</p> </li><li> <p>Provide outreach and information to community members/patients in regards to ACP.</p> </li><li> <p>Address/correct incorrect POA-HC documents.</p> </li><li> <p>Facilitate team member education at various ThedaCare locations.</p> </li><li> <p>Facilitate patient/community informational sessions at various ThedaCare locations.</p> </li><li> <p>Schedule patients directly with ACP resources within the system.</p> </li><li> <p>Support ACP Facilitators throughout system through education resources.</p> </li></ul> <p>Qualifications</p> <ul> <li> <p>Bachelors or Masters degree in social work from an accredited university.</p> </li><li> <p>Licensed/Certified as Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Independent Social Worker (CISW), or Certified Social Worker (CSW)</p> </li><li> <p>Certified as a Respecting Choices ACP Facilitator (or willing to complete within the first six months of employment)</p> </li><li> <p>Experience in health care preferred.</p> </li></ul> <p>Physical Demands</p> <ul> <li> <p>Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance</p> </li><li> <p>Category 2: Job classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job duties</p> </li></ul> <p>Work Environment</p> <ul> <li> <p>Normally works in climate controlled office environment</p> </li><li> <p>Frequent sitting with movement throughout office space</p> </li></ul> <p>Scheduled Weekly Hours:</p> <p>40</p> <p>Scheduled FTE:</p> <p>1</p> <p>Location:</p> <p>ThedaCare Regional Medical Center - Appleton - Appleton,Wisconsin</p> <p>Overtime Exempt:</p> <p>No</p>
