Demonstrates a mastery of basic skills of an oncology pharmacist. Maintains an acquired body of knowledge with experience gained in order to have a positive impact on the care of the oncology patient. Demonstrates clinically sound decision making skills.</p> <p>EEO/AA/Disability/Veteran</p> <p>Responsibilities</p> <ul> <li> <ol> <li>Resolves drug distribution problems assuring accurate and timely dose delivery to all patients </li></ol> </li><li>1.1 Responds to telephone requests for medications in appropriate fashion accommodating callers request or routing call to appropriate person </li><li> <ol start="2"> <li>Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans </li></ol> </li><li>2.1 Demonstrates knowledge of basic pharmacokineteic principles and pharmacokinetic calculations </li><li> <ol start="3"> <li>Problem Identification and Resolution </li></ol> </li><li>3.1 Communicate and documents problem detection, relevant findings, recommmendations,and necessary follow-up </li><li> <ol start="4"> <li>Provides drug information to nurses, prescribers, and other healthcare professionals </li></ol> </li><li>4.1 Serves as primary source of drug information to physicians, nurses, and other healthcare professionals </li><li> <ol start="5"> <li>Educational Activities </li></ol> </li><li>5.1 Participates in and contributes to educational forums within the department (written or verbal) </li><li> <ol start="6"> <li>Documents medication use variances (MUVs) and Adverse Drug Reactions (ADR's) </li></ol> </li><li>6.1 Documents actual and prevented medication use variances within the pemenic system </li><li> <ol start="7"> <li>Complete department education and competancies in a timely manner in order to satisfy regulatory requirements </li></ol> </li></ul> <p>Qualifications</p> <p>EDUCATION</p> <p>Graduated from an accredited College of Pharmacy with a B.S. or Pharm. D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.</p> <p>EXPERIENCE</p> <p>Two years experience in an oncology based hospital or outpatient oncology pharmacy setting is preferred for this position</p> <p>LICENSURE</p> <p>Licensed to practice pharmacy in Connecticut or eligible for reciprocation</p> <p>SPECIAL SKILLS</p> <p>This individual should demonstrate knowledge of oncology pharmacology and therapeutics, and understand pharmacy systems of drug distribution, manufacturing technology, clinical trials when applicable, and formulary management.
