<p>The Cincinnati VA Medical Center is seeking a Registered Nurse to work in our Home-Based Primary Care (HBPC) Program at the Norwood CBOC Location. The RN will assess, plans, implement, and evaluate care based on age-specific components. They will assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program. Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.</p> <p>Major Duties may include but not limited too:</p> <ul> <li>Assesses, plans, implements, and evaluates care based on age-specific components. </li><li>Administers medications and procedures per established policies and guidelines. </li><li>Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. </li><li>Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. </li><li>Makes home visits to Veterans in the program </li><li>Focuses on individual growth and development in HBPC nursing practice with increased levels of self-direction. </li><li>Seeks guidance on how to improve care provided to patients and families. </li><li>Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. </li><li>Brings HBPC principles and theory with limited application of critical thinking skills. </li></ul> <p>Preferred Experience: Case Manager Home care experience preferably with an emergency and/or ICU experience.</p> <p>Supervisory Controls: The Registered Nurse is responsible to the assigned rating and approving supervisors and is accountable to the Associate Director of Patient Care Services (ADPCS).</p> <p>Work Schedule: Monday - Friday 7:30am - 4:00pm</p> <p>Virtual: This is not a virtual position.</p> <p>Relocation/Recruitment Incentives: Not Authorized</p> <p>Permanent Change of Station (PCS): Not Authorized</p>
