<p>Position Summary Collaborates with the interdisciplinary team to coordinate the overall patient care for anyone referred to Hunterdon Health System with a known or suspected diagnosis of breast cancer, facilitating a rapid, efficient transition through the system from referral to diagnosis and treatment; Provides patient education regarding diagnosis and treatment plan; facilitates referrals to appropriate services within and outside Hunterdon Healthcare to meet the physical, psychosocial, financial and spiritual needs of patients. Active member of the breast team and NAPBC accrediation standard complaince.# # Position Responsibilities 1. Adheres to and actively participates with the American College of Surgeons National Accreditation Program of Breast Center standards (including but not limited to data collection and analysis, continuing education, protocol completion and review, survivorship, etc.)## # # Qualifications High School Diploma or Equivalent required Bachelor#s Degree preferred Completion of patient navigation training and certification within 6 months of hire## ### # Minimum Years of Experience 3 - 4 years experience in healthcare is required# Breast oncology experience preferred Patient navigation training and certification preferred # Excellent organization and communication skills (written and verbal); #Self-directed, strong team player, collaborative, calm under stress, resourceful.
