Join our exceptional team and help us continue to provide the highest quality of health care possible to our communities.</p> <p>Mercy Health's Perinatal Outreach program provides intentional social and emotional support to perinatal patients by offering standardized, evidence-based interventions across the continuum of perinatal health care. The Community Health Worker (CHW) address gaps in our current systems by navigating patients through clinical and social systems; advocating, educating, and coordinating services; and ensuring timely, patient-centered, unbiased, culturally appropriate care. The CHW will be working in neighborhood settings with a focus on communities experiencing high racial disparities in birth outcomes.</p> <p>Essential Job Functions</p> <ul> <li>Client Enrollment (5%): Assess eligibility and enroll clients in Pathway Programs </li><li>Navigation and Coordination (40%): Coordinate and follow-up on referrals to other community organizations that specialize in areas prioritized by individuals. Referrals may include comprehensive and facilitated referrals as well as application assistance and client advocacy. Provide basic family and individual facilitation; work with individuals to develop prioritized pathways. Coordinate with organizations to improve access to services for clients by directly assisting clients in obtaining transportation to appointments and conducting home visits. </li><li>Patient Education (15%): Provide current and appropriate general health and wellness education throughout interaction with clients as well as teaching self-advocacy skills. </li><li>Documentation and Reporting (15%): Enter client and provider data in the Pathways database per the program protocol, including demographics, eligibility/enrollment status, and referral status. Identify and document system/service barriers related to connecting clients with health and social services. Complete required productivity reports. </li><li>Community Education and Advocacy (10%): Serve as community connection point for convening community members. Provide input into the development and implementation of community programs/events that respond to the needs of minority populations with multiple health needs and barriers to care. </li><li>Program Representation and Trainings (15%): Participate actively in community, interagency and coalition meetings to foster good working relationships with the community and other service providers. Participate in regularly scheduled team and inter-organizational meetings and trainings. </li></ul> <p>Required Minimum Education:</p> <p>High School Diploma or GED</p> <p>Licensure/Certification Preferred:</p> <p>Ohio CHW certificate preferred. CHW certification is required within 12 months of hire, which would be supported through the program.</p> <p>Minimum Years and Type of Experience:</p> <p>3 years</p> <p>Other Knowledge, Skills and Abilities Required:</p> <p>Sensitivity and experience in working within different cultures. Ability to communicate orally and in writing in English; fluent in a second language desired. Proficient with computers and accuracy with data entry and Microsoft Office. Ability to work independently or with little supervision. Ability to separate personal from professional interactions with clients and maintain professional/ethical boundaries. Ability to document client interactions with accuracy and in a timely manner. Ability to learn and implement new procedures and adapt to emerging community needs. Valid Ohio driver's license with one year driving experience and no traffic citations.
