Horeb / Monticello</p> <p>Based in Mt Horeb| Daily travel throughout the county is required</p> <p>Full-time, Benefit Eligible | 40 hrs/week</p> <p>Community-Based Role with Regional Travel</p> <p>Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated and compassionate HUD Service Coordinator to support residents across 3 properties in Mt. Horeb, Monticello, and Madison, WI.</p> <p>This role is designed to promote independence and self-sufficiency for individuals residing in HUD-supported properties. You'll connect tenants with vital community services, reduce early admissions to assisted living, and help residents maintain their independence for as long as possible.</p> <p>What You'll Do</p> <ul> <li>Provide informal case management, intake, and referrals </li><li>? Assess health, psychological, and social needs of tenants </li><li>Develop and monitor individualized service plans </li><li>Build relationships with local service providers and maintain a resource directory </li><li>? Educate tenants on available services, rights, and application processes </li><li>Present workshops and training sessions on topics of interest </li><li>Foster informal support networks among residents, families, and volunteers </li><li>Collaborate with property staff and educate on aging-in-place strategies </li><li>Maintain accurate documentation and case records </li></ul> <p>?
