It is not designed to contain or be interpreted as a comprehensive inventory of all duties, responsibilities and qualifications required of employees assigned to the job.</p> <p>Essential Functions</p> <p>Patient Navigation:</p> <ul> <li>Serve as a bridge between the community and the dental clinic, helping refugee and other vulnerable population patients access dental services. </li><li>Assist patients with appointment scheduling and transportation arrangements. </li><li>Assist patients with navigating financial resources and assistance programs. </li><li>Provides timely documentation into database of all patient interactions, interventions and community resource contacts and communicates contacts to dental physician. </li></ul> <p>Health Promotion and Advocacy</p> <ul> <li>Advocate for the oral health needs of the community, especially for refugee and other vulnerable population patients facing barriers to care. </li><li>Identify systemic issues that affect oral health access and work with the clinic to address them. </li><li>Distribute educational materials on oral hygiene, nutrition, and preventive care both in clinical and community settings. </li></ul> <p>Referrals and Follow-Up:</p> <ul> <li>Provide wrap around service to help patients move between integrated medical and dental clinics and assist patients in making appointments at outside clinics as needed. </li><li>Follow up with patients to ensure they complete recommended treatment plans. </li></ul> <p>This position develops knowledge of relevant community resource agencies and services for refugee and other vulnerable populations. Familiarity with dental needs impacting refugee patients and underserved communities is preferred. This position also builds an understanding of Medicaid and government assistance programs that support refugee and vulnerable populations.</p> <p>Comments</p> <p>Work Environment and Level of Frequency that may be required:</p> <p>Nearly Continuously: Medical office / clinic environment</p> <p>Seldom: Outdoor environment, extreme cold, extreme heat, noise</p> <p>Physical Requirements and Level of Frequency that may be required</p> <p>Nearly Continuously: Sitting, hearing, listening</p> <p>Often: Repetitive hand motion (such as typing), talking, walking, reaching overhead</p> <p>Seldom: Bending, twisting, climbing, kneeling, squatting, balancing, pulling, pushing, lifting up to 50 pounds</p> <p>Minimum Qualifications</p> <p>High School diploma, or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated relationship-building and effective communication skills.</p> <p>This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy.
