<p>Purpose:</p> <p>Directs all operations within assigned territory or facility, either directly or through subordinates, based on patient-driven census modeling. Directly responsible for reputation, financials, including revenue growth, compliance, strategic planning, employee-engagement and development and partnering with organization initiatives.</p> <p>Responsibilities:</p> <p>Care</p> <ul> <li>Participates in developing standards which ensure safe and therapeutically effective service to patients and families. Has joint responsibility with the Administrator for seeing that standards are met. </li><li>Consults with the Executive Director of Hospice to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of personnel. </li><li>Participates in establishing the functions and qualifications for each service position; Coordinates interdisciplinary team services. </li><li>Assures ongoing assessment of patient/family needs and supervises development, implementation and evaluation of interdisciplinary group plan of care. Assures physician approval of plans for continuity of medical care. </li><li>Organizes hospice to delineate and delegate authority, functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service. </li><li>Oversees confidentiality and safeguards all patient-related information in compliance with HIPAA regulations. </li><li>Investigates and reports any problem relating to patient care or conditions which might harm the patient and/or employee well-being. Addresses and corrects all patient concerns. </li><li>Immediately reports any accident, incident, lost articles or unusual occurrence to the Executive Director of Hospice. </li><li>Oversees hospice's ongoing Quality Assessment/Performance Improvement (QAPI) program. </li><li>In collaboration with peers and Administrator On-Call, provides 24 hours/day, 7 days/week on-call coverage. </li><li>Conducts patient case conferences, inservices, staff meetings and maintains documentation; Participates in community education projects. </li><li>Teaches, supervises and counsels the hospice patient and family members about providing care for the patient. </li><li>Maintains skills and knowledge. </li></ul> <p>Staff</p> <ul> <li>Provides supervision of clinical, support and clerical employees, and oversees staffing and assignments. </li><li>Assists in education/training programs. Participates in planning for the orientation of new employees. Participates in orientation of new personnel as appropriate to team responsibilities. </li><li>Displays a willingness to support policies and procedures and uses appropriate channels for change of such policies. </li><li>Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance. </li><li>Serves as a resource person to employees. Plans and arranges for consultation needs of staff. Provides individual or group support concerning job related stress or issues. </li><li>Develops a cooperative relationship and communicates effectively and professionally with the physicians. </li><li>Conducts appropriate supervisory visits which allow for clinical observation of performance of assigned team members. </li><li>Provides information for inclusion in written performance evaluations for all staff assigned to team in cooperation with appropriate members of the management team. </li><li>Identifies needs for staff education and assists in the development of inservice programs for interdisciplinary group. </li></ul> <p>Business</p> <ul> <li>Responsible for seeing that objectives are implemented. </li><li>Makes routine, scheduled calls on referral sources for the purpose of promoting access to hospice and referrals to the organization. </li><li>Participates in coordinating hospice's services with services of other community agencies. </li><li>Accepts responsibility for regular attendance and punctuality, fulfills job requirements without regard to time involved. </li><li>Works with Executive Director of Hospice in identifying budgetary requirements and determining appropriate use of allocations. </li><li>Monitors equipment for appropriate use and takes steps to keep misuse to a minimum. </li><li>Ensures that all necessary supplies and equipment are available. </li><li>Is normally available at all times during and after operating hours; may designate a qualified temporary replacement if he/she will not be available. </li><li>Serves as a member of the management team in order to address problems associated with the delivery of services and staff development needs, to put forth suggestions for appropriate additions/changes in policies and procedures and to give peer input to enhance and improve the management function. </li><li>Supervises the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards. </li><li>Performs specific assignments as directed by the Executive Director of Hospice. </li><li>Participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties. </li></ul> <p>Qualifications:</p> <ul> <li>Licensed RN, or bachelor's degree in a recognized discipline of the IDG, i.e., RN, SW, SCC, BC, or master's degree in nursing, social work, counseling, health care administration, public administration, business </li><li>At least 1 year in a leadership position </li><li>Knowledge of professional principles and practices for patient care disciplines dealing with end-of-life care </li><li>Knowledge of Medicare and Medicaid conditions of participation and standards for hospice and other health care entities </li><li>Demonstrated knowledge of current leadership and supervisory principles and practices with strong human relation skills; Experience working in a team setting </li><li>Demonstrated knowledge of continuous quality improvement environments and techniques </li><li>Ability to develop and maintain business relationships including good public contact and group presentation skills </li><li>Demonstrated ability to handle multiple priorities in an organized manner, make timely decisions and exercise sound independent judgment </li><li>Ability to evaluate people and provide an environment that motivates others </li><li>Demonstrated effective verbal and written communication skills with individuals and groups </li><li>Demonstrated ability to identify problems/issues and confront them; Effectively employ negotiation, conflict management and quality improvement skills to resolve issues </li><li>Strong keyboarding and computer skills and the ability to learn work-related software </li><li>Current American Heart Association Healthcare Provider certification (within 30 days of hire) </li><li>Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire) </li><li>Must have valid driver's license, auto liability insurance and reliable transportation if driving is required </li><li>Successful completion of pre-employment physical, drug screen, 2-step TB test and background check </li></ul> <p>Working Conditions:</p> <ul> <li>Physical requirements: Sedentary work exerting up to 10 pounds occasionally, and/or a small amount of force frequently to move objects, including the human body </li><li>Physical activities: Talking, Hearing, Repetitive Motions, Fingering (picking, pinching, typing, etc.), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations) </li><li>Environment: Work is primarily performed in a climate-controlled environment with minimal safety/health hazard potential </li><li>Time: Ability to be flexible with a varying schedule and programmatic requirements </li><li>Travel: Required between locations </li></ul> <p>These characteristics are representative of those an employee encounters while performing the essential functions of this job.
