<p>General Summary/Responsibilities:</p> <p>Department: Social Services</p> <p>Shift: 8:00 AM-5:00 PM</p> <p>Status: [Full-Time] 80 h per pay period</p> <p>Facility: Miami Valley Hospital</p> <p>Responsible for overseeing and enhancing the continuum of care for patients. This role involves coaching, supervising, and mentoring social work team members to achieve optimal patient care outcomes.</p> <p>The manager will collaborate with interdisciplinary teams and external stakeholders to address process efficiencies and ensure high-quality care during transitions in the acute care setting.</p> <p>Provides direct supervision for the overall daily operations of social work staff that include but is not limited to technical responsibilities, staffing, payroll, work unit management, education, after-hour coverage, team facilitation, and professional development.</p> <p>Essential Duties & Functions:</p> <p>Lead and manage social work services by mentoring and guiding team members to achieve patient-centered goals.</p> <p>Collaborate with the Integrated Care Management (ICM) Director, ICM Managers, and multi-disciplinary care team members to optimize process efficiency related to patient and family care.</p> <p>Develop and outline best practices within the medical social work field to enhance service delivery.</p> <p>Assist in creating and managing programs that address medical social work needs across the continuum of care.</p> <p>Participate actively in the development, implementation, and evaluation of departmental goals and objectives, aligning them with organizational aims such as throughput optimization, readmission prevention, health equity, employee engagement, and improved patient experience.</p> <p>Serve as a financial steward for the organization by implementing operational efficiencies that improve resource allocation, ensuring sustainability while upholding high-quality patient care.</p> <p>Serve as a consultant and represent social work services in discussions with external groups and agencies to promote best practices and service initiatives.</p> <p>Provide leadership by supervising care team members, offering educational opportunities, and fostering professional growth within the team.</p> <p>Evaluate and monitor department operations in conjunction with the ICM Director to identify and implement opportunities for improvement.</p> <p>315-Licensed Independent Soc Wrkr MM-Master's Degree</p> <p>Education</p> <p>Minimum Level of Education Required: Master's degree</p> <p>Additional requirements:</p> <p>§ Type of degree: Social Work from an accredited School of Social Work</p> <p>§ Area of study or major: Medical Social Work</p> <p>§ Preferred educational qualifications: N/A</p> <p>§ Position specific testing requirement: N/A</p> <p>Licensure/Certification/Registration</p> <p>§ Current Ohio Social Work licensure required.</p> <p>§ LISW required.</p>
