(for CBOCS)</p> <p>The New Mexico VA Health Care System (NMVAHCS) Ambulatory Care Service (ACS) is seeking qualified applicants to join our team. We are seeking a full-time Nurse Practitioner with prescriptive authority to function as part of ACS - Primary Care, Main Campus (Farmington, Gallup, Raton, Silver City - CBOCS)</p> <p>Multi Disciplinary Position - Physician Assistant Announcement # 12828596</p> <p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p> <p>The incumbent serves as a Nurse Practitioner (NP) practices in the Ambulatory Care Service, Main Facility, to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.</p> <p>The NP practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position.</p> <p>The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.</p> <p>Duties include but are not limited to:</p> <ul> <li>Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. </li><li>Develops and implements an individualized plan to achieve optimal population health and disease management. </li><li>Demonstrates leadership and works collaboratively with the interdisciplinary team. </li><li>Orders and interprets labs, tests, and diagnostic studies. </li><li>Prescribes pharmacologic and non-pharmacologic interventions. </li><li>Initiates referrals and consults to specialty services. </li><li>Provides education to patients and their families. </li><li>Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. </li><li>Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. </li></ul> <p>Work Schedule: Full Time, Monday - Friday, 7:30 am - 4:00 pm (Work schedules are determined and approved by the supervisor)</p> <p>Telework: Not Available</p> <p>Virtual: This is not a virtual position.</p> <p>Relocation/Recruitment Incentives: May be Authorized (CBOCS)</p> <p>Education Debt Reduction Program (EDRP): May be Authorized</p> <p>Permanent Change of Station (PCS): Not Authorized</p> <p>Financial Disclosure Report: Not required</p>
