<p>Job Description:</p> <p>We are seeking a dynamic and motivated liaison to join our team. In this role, you will serve as a</p> <p>key representative for the Clinically Integrated Network (CIN), collaborating with the Medical</p> <p>Director and Director of Quality and Care Management to develop and maintain strong</p> <p>relationships with member physician practices.</p> <p>Day-to-Day Responsibilities:</p> <p>? Practice Engagement: Visit physician practices regularly, adjusting frequency based</p> <p>on priority and specialty (Primary Care: regular meetings and pop-in visits; Specialists:</p> <p>ad hoc meetings).</p> <p>? Training & Education: Conduct training on our population health management</p> <p>platform, Lightbeam, and provide education to clinic staff on CIN priorities and quality</p> <p>initiatives.</p> <p>? Performance Reviews: Assist in monthly meetings with providers who require a</p> <p>performance remediation plan, ensuring quality and cost goals are met.</p> <p>? Resource Management: Maintain a library of resources to share with practices to</p> <p>support value-based efforts and quality improvement.</p> <p>? Quality Measure Improvement: Collaborate with clinic nurses and medical staff to</p> <p>streamline processes that improve quality measure performance.</p> <p>? Communication: Update physician offices on key changes from payors and CIN to</p> <p>ensure practices remain informed and aligned with industry standards.</p> <p>? Collaboration: Work closely with the Director of Quality and Care Management on the</p> <p>implementation and accomplishment of key quality and cost objectives.</p> <p>Qualifications:</p> <p>? Bachelor's degree required</p> <p>? Previous experience in healthcare, quality improvement, or population health</p> <p>management preferred</p> <p>? Strong interpersonal skills with the ability to work effectively with a wide range of</p> <p>stakeholders, including physicians, administrators, and healthcare staff</p> <p>? Ability to work autonomously and manage time effectively while prioritizing task</p>
