The primary role of the Nurse Coordinator is to maintain communication with patients, families, staff, and providers in order to ensure coordination of patient care across the continuum.</p> <p>EEO/AA/Disability/Veteran</p> <p>Responsibilities</p> <ul> <li> <ol> <li>Clinical Practice </li></ol> </li><li>1.1 Manages incoming referrals, coordinates initial patient contact, and compiles list of presented patients in collaboration with attending physicians, fellows, and Advanced Practice Provider, (APP's). </li><li> <ol start="2"> <li>Clinical Program </li></ol> </li><li>2.1 Assists in the coordination of the Orthopedic program. </li><li> <ol start="3"> <li>Patient Safety, Quality, and Research </li></ol> </li><li>3.1 Creates and supports a culture and environment of safety. </li><li> <ol start="4"> <li>Professional Development and Education </li></ol> </li><li>4.1 Participates in enhancing the professional development of the nursing staff and other health care providers involved in the care of Orthopedic patients. </li></ul> <p>Qualifications</p> <p>EDUCATION</p> <p>Bachelor's Degree in Nursing required. Connecticut RN license</p> <p>EXPERIENCE</p> <p>A minimum of three (3) years experience in adult orthopedic acute care inpatient services, ambulatory or critical care.</p> <p>LICENSURE</p> <p>RN license in the State of Connecticut</p> <p>SPECIAL SKILLS</p> <p>Knowledge of orthopedic procedures and surgeries.
