In this impactful role, you will be instrumental in providing support to individuals within our supportive housing program.</p> <p>POSITION RESPONSIBILITIES</p> <ul> <li>With Administrator, assists residents with the development and implementation of a long-range service plans </li><li>Assists in securing and maintaining entitlements: SSI/SSD, PA Medicaid, SNAP, Section 8 housing, etc. </li><li>Conducts weekly home visits and one phone contact with assigned residents and additional support to resident(s) in crisis as needed and provides appropriate documentation of all contact </li><li>Responsible for contact and documentation of such with collateral service providers at least once a month and as needed </li><li>Assesses and provides referrals to residents regarding any unmet service needs </li><li>Attention to providing and modeling the necessary development of ADL skills: budgeting, cooking, household maintenance, personal hygiene, use of public transportation, etc. </li><li>Assists in the transportation of residents to MH programs and events </li><li>Responsible for assuring good relationships with neighbors and the general community </li><li>Ensures that room/apartments are clean for new residents (may include cleaning, moving and setting up) </li><li>Responsible for reporting all housing maintenance issues through the appropriate procedure </li><li>Responsible for the accurate and current maintenance of phone log, in addition to resident records </li><li>Immediately notifies Supervisor of all potential crisis/emergency situations </li><li>Documents all incidents and occurrences </li><li>Conducts resident house meetings, as needed </li><li>Conducts fire drills at each residence bi-monthly </li><li>Responsible for assisting residents at the time of moving in/out </li><li>Meets weekly for supervision </li><li>Reviews residents rent monthly with Supervisor </li><li>Attends and participates in staff meetings and scheduled in-service training and/or additional educational opportunities as well as appropriate agency and interagency meetings </li><li>With Supervisor, responsible for discharge planning of residents </li><li>Acts as task supervisor when interns are assigned to Supported Housing Program </li><li>Responsible for Agency compliance requirements by grants through site visits, data collection, and tracking system </li><li>Rotation of on-call duties </li><li>Responsible for other related activities </li></ul> <p>POSITION REQUIREMENTS</p> <ul> <li>High School Diploma or Equivalency with experience in human services required </li><li>Bachelor's Degree in Human Services or related field preferred </li><li>Experience working with individuals with severe mental illness preferred </li><li>Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle </li><li>Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle </li></ul> <p>POSITION RELATIONSHIPS</p> <ul> <li>Reports to: Community Services Coordinator </li><li>Supervises: N/A </li><li>Training takes place Monday - Friday 8:30am - 4:30pm* </li></ul> <p>SHIFT: Sunday through Thursday (8:30 am to 4:30 pm), with an on-call availability</p> <p>PAY RATE: $21.00 per hour</p> <p>LOCATION: Main Office in Islip Terrace, NY</p> <p>If you're passionate about making a difference and contributing to our mission, we want to hear from you.
