<p>Job Summary and Responsibilities</p> <p>As a Diabetes Educator Dietitian, you will provide specialized nutrition education and counseling for individuals with diabetes, empowering self-management.</p> <p>Every day you will assess patient needs, develop personalized meal plans, educate on glucose management and lifestyle, and conduct sessions applying evidence-based practices.</p> <p>To be successful, you will demonstrate expert knowledge in diabetes nutrition, excellent teaching skills, and a compassionate approach to optimize patient health outcomes.</p> <ul> <li>Coordinates the planning, implementation and evaluation of the DSMES program. </li><li>Measures the effectiveness of the DSMES through the development and utilizations of a Continuous Quality Improvement (CQI) plan. </li><li>Responsible for maintaining ADA Recognition and participating in the evaluation of the program. </li><li>Develops and maintains an education curriculum, including selection and on-going review of educational materials to ensure they meet the needs of the population targeted. </li><li>Develops and maintains policies/procedures/standing orders as they relate to inpatient and outpatient education. </li><li>Provides education following the ADA curriculum framework of these 9 topic areas: diabetes pathophysiology; healthy eating; being active; taking medication; monitoring glucose; acute complications - prevention, detection and treatment; chronic complications - prevention, detection and treatment; lifestyle and healthy coping; and diabetes distress and support for both inpatients and outpatients. </li></ul> <p>We are offering a $10,000 Incentive to new hires who meet the eligibility requirements.</p> <p>Job Requirements</p> <p>Required</p> <ul> <li>Masters Other Social Work, upon hire or </li><li>Associates Of Science Nursing, upon hire or </li><li>Bachelors Of Science Nutrition, upon hire or </li><li>Doctoral degree of Pharmacy, Psychology, Optometry, Physical Therapy, Medicine, Podiatric Medicine, Physician Assistant Studies or Occupational Therapy, upon hire or </li><li>Masters Other Occupational Therapy, upon hire or </li><li>Bachelors Of Science Nursing, upon hire or </li><li>Bachelor's degree in exercise physiology, exercise science, kinesiology or another healthcare and related field, upon hire or </li><li>Bachelors Other health education or health promotion, upon hire </li><li>Dietitian: ND, upon hire and </li><li>Registered Dietitian CDR, upon hire and </li><li>Certified Diabetes Educator, upon hire </li></ul> <p>Where You'll Work</p> <p>CHI St. Alexius Health Dickinson, part of CommonSpirit Heath, a national nonprofit health system based in Englewood, Colorado. The faith-based system operates in 18 states and includes 103 hospitals. Additional services offered within the system are: long-term care, assisted and residential living communities, community health services organizations, home health agencies, and numerous outpatient facilities.</p>
