<p>Empath Health is currently seeking a Licensed Practical Nurse (LPN) to join our Continuous Care (CC) team.</p> <p>Three 12 hour shifts with alternating weekends - days to be determined. 8pm - 8:30am. This position covers our 4 county service area including Manatee, Sarasota, Charlotte and DeSoto.</p> <p>Under physician's orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.</p> <p>Primary tasks, duties, and responsibilities:</p> <ol> <li> <p>Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.</p> </li><li> <p>In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.</p> </li></ol> <p>Requirements:</p> <ul> <li>Applicants must be a graduate of an approved nursing program and own a current Florida LPN license. </li><li>Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred. </li></ul> <p>Empath Health values diversity as it strengthens our community and care.
