<p>Purpose:</p> <p>The Justice Services Case Manager is responsible for supporting client treatment goals by providing connection to resources in our community and assisting in the coordination of treatment for the client's treatment episode.</p> <p>Job Summary:</p> <p>Active participation on multidisciplinary, multi-agency/department treatment team to assist clients in identifying, securing, and sustaining necessary resources to aid in their recovery.</p> <p>Essential Functions:</p> <ul> <li>Assist clients in skill building and resource development in order to successfully live as independently as possible </li><li>Provide a variety of case management services, under supervision, assisting clients to apply for various types of public assistance and providing supporting documentation, attending multi-agency clinical case consultations, and facilitating referrals to other organizations </li><li>Support client recovery by developing and enhancing client access to resources for vocational, financial, medical, housing, utilitites, food, transportation or basic living needs as appropriate </li><li>Work collaboratively with clients, staff, and community partners to provide assessment, develop treatment plans and coordinate appropriate care. </li><li>Assist clients in crisis situations to provide appropriate interventions or referrals </li><li>Accompany clients to medication evaluations and medical services as needed </li><li>Complete documentation of services, assessments, evaluations and treatment plans consistent with clinical and administrative policies and procedures </li><li>Facilitate groups, classes and workshops to support treatment goals </li><li>Comply with HIPAA and other Federal or State Data Privacy laws related to the handling of organizational data. </li><li>Meet defined individual and department goals, activity metrics and Key Performance Indicators. </li><li>Promote, support and demonstrate MHP's mission, vision and values through both behavior and job performance and adheres to policies, procedures, Compliance Plan and Standards of Conduct on a day-to-day basis. </li><li>Effectively interact and communicate with other CFHW staff/clients/customers/partners/etc. </li><li>Other duties as assigned. </li></ul> <p>Specialized Duties:</p> <p>Justice Services</p> <ul> <li>Specialized ongoing caseload comprised of clients currently on probation and/or porole requiring theraputic treatment </li><li>Assists in developing plans of tranistion for clients to successfully transition back into community from jail/prison while achieving mental wellness </li><li>Works collaboratively with internal and external partners such as: Probation, Public Defender, District Attorney, law enforcement, monitoring agencies, and other treatment providers to assist clients in their recovery process and to ensure the continuity and quality of care </li></ul> <p>What's In It For You</p> <ul> <li>Full-time job with benefits (medical, dental, vision, life, disability and retirement plan) </li><li>Paid time off (26 days in total Jan-December in Vacation, Personal Days and Sick Days) and paid holidays (10 holidays) </li><li>Engaged employer who believes you are an essential factor in delivering our mission to the community </li><li>Flexible schedule </li><li>Training: personal and professional growth opportunities </li><li>A team that will support you, and ask you to jump in to lend your </li></ul> <p>What we need for this job:</p> <ul> <li>Bachelor's Degree in Psychology, Social Work, Counseling, or related field preferred or one-year experience in the behavioral health field for non-related Bachelor's degree </li><li>Experience working with adults and/or children in an outpatient setting </li><li>Provide some community based services to enhance skills and promote community integration as needed </li><li>Must have current CO Driver's License and a safe driving record </li><li>Attend regular staffings as needed for clients, including as needed meetings in criminal justice settings such as court or jail. </li></ul> <p>As a condition of employment, you will be required to receive the COVID-19 vaccination (and any subsequent boosters) and the annual influenza vaccine.
