The Nurse will assess, evaluate, and prioritize urgency of medical needs through telephone communication with patients following established standards, practices, and protocols.</p> <p>This is a full time, 40 hours per week position. Monday through Friday 8 am to 5 pm with 1 hour lunch break. After 90 days successful probationary period this position could possibly go to a hybrid position.</p> <p>This nurse position will be supporting our Family Practice department. Prefer experience with all ages, OB or Labor & Delivery experience and triage experience.</p> <p>Responsibilities (How this position enhances our Mission, Vision, and Values)</p> <p>Mission - Improve the Quality of Life of Others:</p> <ul> <li>Triages calls with the patient, to determine the appropriate level of care necessary coordinates with care teams and schedules appointments accordingly. </li><li>Schedules appointments for new and returning for all health related provides, excluding Family Planning and WIC. </li></ul> <p>Vision - ODHS preferred partner in East Central Indiana for improving the well-being of individuals and the communities in which they live:</p> <ul> <li>Observes, records, and reports to physician/nurse practitioner, patient's condition and reaction to drugs and treatments as indicated. </li><li>Provides patient education relating to current conditions as appropriate. </li><li>Consults with Chief Medical Officer and/or Director of Clinical Operations regarding unresolved client/patient problems. </li></ul> <p>Values - Esteem the Team, Maximize Potential, Do the Right Thing, Choose Compassion, Encourage Uniqueness:</p> <ul> <li>Implements the nursing process and communicates with the physician/nurse practitioner regarding concerns and/or problems regarding the client/patient. </li><li>Maintains and reviews patient's records, charts, and other information. </li><li>Maintains flexibility nursing assignments to include, but not limited to, follow-up, chart audits, and on floor nurse when needed. </li><li>Develops and maintains ongoing knowledge of nursing policies and procedures and standing orders and implements as appropriate. </li><li>Participates in professional development activities and maintains professional affiliations. </li><li>Attends required meetings and in-services and participates on committees as requested. </li><li>Reports diagnostic results to the identified team member within an identified timeframe. </li><li>Maintains patient confidentiality. </li><li>Performs all other duties as assigned. </li></ul> <p>Education and Experience:</p> <ul> <li>Minimum graduate of an accredited school of licensed practical nursing. </li><li>Prefer two years of professional nursing experience in clinic/office setting. </li><li>Prefer 1-2 years of telephone triage nurse responsibilities. </li><li>Possession of an Indiana State Nurse license. </li><li>CPR certification and TB certification, required to be maintained through ODHS post hire. </li></ul> <p>About you:</p> <ul> <li>Ability to react calmly in emergency situations. </li><li>Ability to read, understand, and follow detailed written and oral communication. </li><li>Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care. </li><li>Knowledge of organizational policies, regulations, and procedures to administer patient care and common safety hazards and precautions to establish a safe work environment. </li><li>Skill in establishing and maintaining effect working relationships with patients, medical staff, and the public. </li><li>Skill in initiative, listening, judgment, discretion, and decision-making within the guidelines of the policies, procedures and standing orders of the Health Center. </li><li>Skill in preparing and maintaining records, charting, and writing reports. </li><li>Ability to prioritize work assignments, work independently, and perform duties efficiently. </li><li>Understanding of individual's socio-economic status and lifestyle with respect for individual differences. </li><li>Supports ODHS outreach events. </li><li>Supports other departments as needed & applicable. </li></ul> <p>Physical and Environmental Requirements:</p> <ul> <li>Frequent exposure to communicable diseases, toxic substances, medicinal preparations, and other conditions common to clinic environment. Tasks involve contact with blood or body substances. </li><li>Requires standing, walking or sitting for long periods of time. Occasional bending, stretching or lifting up to 40 pounds. </li><li>Requires the ability to work under stressful situations. </li><li>Requires corrected vision and hearing to normal range or use of approved adaptive equipment. </li></ul>
