<p>Job Summary</p> <p>Responsible for supporting social work and case coordination activities between ambulatory and acute care.</p> <p>Manage referrals from ambulatory offices for patients who need social service assistance, including but not limited to transportation, medication assistance, home health referrals, long term care placement.
