<p>Job Summary</p> <p>Responsible for supporting social work and case coordination activities between ambulatory and acute care.</p> <p>Manage referrals from ambulatory offices for patients who need social service assistance, including but not limited to transportation, medication assistance, home health referrals, long term care placement. Discharge follow-up calls and coordination of additional services to address social determinants of health.</p> <p>Responsibilities</p> <ul> <li>Obtain information and answer questions concerning physicians' and patient-specific needs, </li><li>Maintain medical terminology skill level needed to discuss and understand clinical information with physician offices, customers, and hospital clinical staff, </li><li>Process inbound calls/electronic referrals for social service needs, </li><li>Properly route to Social Worker/Case Manager based upon established protocol, </li><li>Perform data entry as well as document in the electronic health record, </li><li>Responsible for handling telephone calls from multiple ambulatory locations and routing to Social Worker/Case Manager, as per protocol, </li><li>Generate, review and sort daily reports. </li><li>responding to referrals from ambulatory offices within the Vandalia Health network to the Resource Center for Case Management, for social service requests. </li><li>Assist CM team with timely submission of home health, DME, and long-term care referrals, via electronic means </li><li>advancing referrals to Social Worker/Case Manager, as per protocol </li></ul> <p>Knowledge, Skills & Abilities</p> <p>Experience/Skills</p> <ul> <li>Excellent data entry skills </li><li>Excellent phone/customer service skills </li><li>Experience with Cerner (or other electronic health records) </li><li>Knowledge of insurance (payor) requirements </li><li>Medical terminology </li><li>Ability to multi-task </li><li>Knowledge of Social Determinants of Health </li><li>Knowledge of community-based services </li></ul> <p>Working Conditions</p> <p>Essential:</p> <ul> <li>Arm Coordination </li><li>Crouching and Stooping </li><li>Hand Coordination </li><li>Ability to Hear </li><li>Kneeling </li><li>Lifting Irregular Loads </li><li>Squatting </li><li>Ability to see </li><li>Ability to sit for 8 hours </li><li>Ability to walk up to 1 hour </li><li>Ability to lift up to 25 lbs </li><li>Ability to pull up to 10 lbs </li></ul> <p>Patient Group Knowledge (Only applies to positions with direct patient contact)</p> <p>Must demonstrate competency through an initial orientation and ongoing competency validation to independently perform tasks and additional duties as specified in the job description and the unit/department specific competency checklist.</p> <p>Common Duties and Responsibilities</p> <p>(Essential duties common to all positions)</p> <ol> <li> <p>Maintain and document all applicable required education.</p> </li><li> <p>Demonstrate positive customer service and co-worker relations.</p> </li><li> <p>Comply with the company's attendance policy.</p> </li><li> <p>Participate in the continuous, quality improvement activities of the department and institution.</p> </li><li> <p>Perform work in a cost-effective manner.</p> </li><li> <p>Perform work in accordance with all departmental pay practices and scheduling policies, including but not limited to, overtime, various shift work, and on-call situations.</p> </li><li> <p>Perform work in alignment with the overall mission and strategic plan of the organization.</p> </li><li> <p>Follow organizational and departmental policies and procedures, as applicable.</p> </li><li> <p>Perform related duties as assigned.</p> </li></ol> <p>Education</p> <ul> <li>Associate's Degree (Required) • Associate Degree in related field, with 1 year related experience required, or • Certified Medical Assistant plus 2 years related experience, or • Substitution: High School Diploma and 3 years-experience in a Healthcare setting (Hospital, Clinic, Home Health, Skilled Nursing Facility, or Physician office) • Substitution: Peer Recovery Support Certification plus 1 year experience </li></ul> <p>Credentials</p> <p>Work Schedule: Days</p> <p>Status: Full Time Regular 1.0</p> <p>Location: General Hospital</p> <p>Location of Job: US:WV:Charleston</p> <p>Talent Acquisition Specialist: Lisa J.
