This position involves providing consultation and services to homebound individuals, addressing complex social and emotional problems that impede the management of medical regimens.</p> <p>Key Responsibilities:</p> <ul> <li>Conduct comprehensive assessments of patients and caregivers to formulate appropriate care plans. </li><li>Develop person-centered plans of care to promote holistic well-being. </li><li>Collaborate with physicians and other health team members to address significant social and emotional factors affecting patients' health. </li><li>Assist patients and families in understanding and following medical recommendations. </li><li>Utilize community resources to help patients resume life in the community or manage disabilities. </li><li>Participate in staff meetings, policy formation, and treatment planning. </li><li>Provide consultative services to Regional Directors and VP. </li><li>Maintain confidentiality of patient, staff, and organizational information. </li></ul> <p>Minimum Qualifications:</p> <ul> <li>Master's Degree in Social Work from an accredited school. </li><li>Valid Social Work License as per state regulations. </li><li>Two years of social work experience in a healthcare setting. </li><li>Ability to perform written, verbal, and observational tasks related to the position. </li><li>Professional demeanor and commitment to patient confidentiality. </li><li>Ability to provide services to acutely and chronically ill patients. </li></ul> <p>Our Commitment to Diversity and Inclusion</p> <p>Trinity Health is one of the largest not-for-profit, Catholic healthcare systems in the nation.
