Please click the apply button and upload your resume to Integrated Primary Care - Behavioral Health Therapist</p> <p>In this role you will:</p> <ul> <li>Create patient-centered treatment plans, provide ongoing treatment and case management services, and assess patient's ongoing clinical needs and progress. </li><li>Perform comprehensive biopsychosocial diagnostic evaluations with patients. </li><li>Provide a range of consultations, short-term, and long-term therapy to be meet the needs of our patients </li><li>Provide a team-oriented approach to patient care and fully participate in integrating behavioral health care with primary care. </li><li>Engage in collaborative crisis management services in urgent situations. </li></ul> <p>The successful applicant will offer:</p> <ul> <li>Must be licensed or license eligible to practice in the Commonwealth of Massachusetts (Psychology, LCSW, LICSW, LMHC or LMFT).
