<p>NURSE CASE MANAGER - DISCHARGE HOSPITAL AMBASSADOR</p> <p>Lynn Community Health Center is a Federally Qualified Community Health Center that provides addiction services for all patients.</p> <p>The Discharge Hospital Ambassador RNCM to serve as an ambassador with 0.5FTE of their time on-site at Salem Hospital and 0.5FTE at LCHC/remote. The RNCM will help ensure seamless follow-up from inpatient to outpatient treatment. The components of this role will include:</p> <p>Hours: 20 hours</p> <p>Location: Salem Hospital</p> <ul> <li>Rounding with the emergency department team to plan discharge follow-up. </li><li>Coordinating with inpatient social work and nurse case management to enhance discharge planning and post-discharge appointments. </li><li>Meeting with the patient's clinical team for discharge planning. </li><li>Meeting with admitted LCHC patients (established or intended) to build engagement in care </li></ul> <p>The successful applicant will offer:</p> <ul> <li>Graduate of an accredited nursing program. </li><li>RN with current Mass.
