We even offer pet insurance for your four-legged family members. </li><li>Early access to earned wages with Daily Pay, tuition reimbursement to help further your career and adoption assistance to help you grow your family. </li></ul> <p>With a collective goal to champion a culture of belonging where everyone feels valued and respected, we honor the ways people are unique and embrace what brings us together.</p> <p>And, we believe equipping you with support and development opportunities is a vital part of delivering an exceptional employment experience.</p> <p>Find a fulfilling career and make a difference with UnityPoint Health.</p> <p>Responsibilities</p> <p>Clinical Excellence</p> <ul> <li>Facilitates the development of the interdisciplinary team plan of care to meet individual spiritual needs. </li><li>Maintain record of spiritual care services and related activities for quality assurance, program development, and policies and procedures. </li><li>Document direct services and ongoing communication with community chaplains, spiritual counselors and hospice interdisciplinary team. </li><li>Attend Interdisciplinary Team meetings and complete all associated documentation in an accurate and timely manner. </li><li>Provide consultation, education, and support to the interdisciplinary group on spiritual care. </li><li>Meets measures as determined and required for job productivity and performance improvement. </li><li>Provide bereavement follow up services as requested. </li><li>Arrange for on-call availability of chaplaincy services. </li><li>Conduct or make arrangements for funeral or memorial services for patients as requested. </li><li>Assist in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team. </li></ul> <p>Collaboration</p> <p>Develop and maintain a resource group of clergy to whom specific aspects of spiritual care may be delegated.</p> <p>Recruit community chaplains, spiritual counselors, and spiritual care volunteers adequately to meet patient/ family/caregiver needs.</p> <p>Develop community contacts and offer education through congregations about hospice care.</p> <p>Provide educational programs for community clergy, religious and lay representatives as resources allow.</p> <p>Provide educational in-services to the hospice and community.</p> <p>Serves as liaison and support to community chaplains and spiritual counselors.</p> <p>*</p> <ul> <li>Performance Improvement </li><li>Completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UnityPoint at Home programs and initiatives. </li><li>Participates in quality assurance and performance improvement activities as assigned. </li><li>Perform other duties as requested by Hospice Administrator or Supervisor to facilitate the smooth and effective operations of the office. </li><li>Maintain regular and consistent attendance at work. </li><li>Behave in a manner consistent with Mission, Vision, Values and Expectations for Excellence. </li><li>Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines </li></ul> <p>Qualifications</p> <p>QUALIFICATIONS:</p> <p>Education:</p> <p>Graduate of accredited school of theology, or related Bachelor's degree.</p> <p>2 units of CPE</p> <p>Experience: Ordination in a particular religious group and/or endorsed and in good standing from an ecclesiastical governing body</p> <p>License(s)/Certification(s):</p> <p>Valid driver's license when driving any vehicle for work-related reasons.</p> <p>Valid Mandatory Reporter course completion by state(s) requirement</p> <p>CPR Certification.</p> <p>Knowledge/Skills/Abilities:</p> <p>Proficiency with computer applications (i.e. Excel, Word, Access)</p> <p>Ability to minister to persons and families in crisis situations</p> <p>Experience in working with patients/families dealing with life-threatening illness and death.</p> <p>Must be comfortable in working in an interfaith setting and able to accept different lifestyles, cultures, beliefs, and values.</p> <p>Strong interpersonal skills.</p> <p>Ability to work as a collaborative team member across service lines as position requires.</p> <p>Ability to understand and apply guidelines, policies and procedures.</p> <p>Able to network with community clergy and congregations.</p> <p>Possesses knowledge of and commitment to the hospice philosophy of care.</p> <p>Other: Use of usual and customary equipment used to perform essential functions of the position</p> <p>#upah123</p>
