<p>Overview</p> <p>Job Summary</p> <p>The family, adult-gero, or acute care nurse practitioner is a professional nurse with advanced knowledge and assessment skills in a specialty are of palliative care. Duties will be performed to assist families with navigating the health care system, to foster seamless rather than fragmented care and to maximize quality of life for patients and their families with life-limiting, chronic illnesses. Duties to be performed in the inpatient and outpatient setting as needed. Demonstrates the role functions of nurse practitioner, consultant, educator, researcher, and leader/change agent. Able to assist with medical and symptom management of the patient.
