<p>RN or LPN Community Based Residence</p> <p>Join our remarkable residential team working in a caring, committed and respectful work environment!</p> <p>Full Platinum Benefits!</p> <p>General Description</p> <p>Support people in their home with intellectual and developmental disabilities (I/DD) to achieve maximum independence integrating agency core values. Provide reliable, compassionate and consistent care while creating meaningful, quality-based relationships in a supportive, cultural environment.</p> <p>Essential Functions</p> <ul> <li>Monitor, assess and advise the management team on health observances and needs, while promoting health and wellness through nursing processes. </li><li>Establish a positive and open communication style with everyone supported including all employees, family members and other health care providers. </li><li>Promote wellness by applying nursing processes. </li><li>Outline all monthly appointments; provide documentation and information needed to successfully complete the appointment. </li><li>When indicated clarify hospital discharge, medication orders, and/or plans of care. </li><li>Participate in hospital discharge or intake process, along with Director of Nursing. </li><li>Provide instructions and education to those supported and staff as it relates to medical diagnoses and healthcare needs (i.e. Allergies, medications, diet, chronic and acute plans of care, etc.) </li><li>Coordinate care with clinics, outside physicians, and hospitals. Attend appointments, as needed. </li><li>Experience with G-tubes, J-tubes, Trach care. </li><li>Other job duties as assigned. </li></ul> <p>Documentation</p> <p>Assist with maintaining medical records for all people supported.</p> <p>Update EMAR. Check and report medication errors, omissions or other concerns to management/RN.</p> <p>Supervise the disposal of discontinued medications (and/or other medical supplies) in accordance with legal requirements.</p> <p>Training</p> <p>Provide training to support staff on people's needs including: competencies, medical history, treatments and wellness plans.</p> <p>Monitor staff progress and assess any need for additional training. Report observations to the management team.</p> <p>Participate in required state and agency trainings, including staff meetings.</p> <p>Provide guidance and support to staff designated to order and stock medications.</p> <p>Additional Functions of the Job</p> <p>Assist with providing coverage for vacancies and peers during vacations.</p> <p>Assumes responsibility for ongoing professional development by participating in community-based educational trainings.</p> <p>All other duties as assigned by supervisor(s).</p> <p>Certification, Licenses, Registrations</p> <ul> <li>Graduate from an accredited school of nursing </li></ul> <p>o RN: Associates or Bachelor's of Science in Nursing.</p> <p>o LPN: Licensed Practical Nurse degree.</p> <ul> <li>Experience providing care for chronic, acute and mental health needs. </li><li>Rhode Island Nursing license </li><li>Proof of COVID vaccination </li></ul> <p>Attitude and Personal Qualities</p> <p>Teamwork/Empowerment - Creates a positive work environment that fosters teamwork. .</p> <p>Respect - Treats everyone with courtesy, fairness and dignity.</p> <p>Integrity/Trust - Committed to the highest standards of personal and professional behavior and is honest and ethical in all actions.</p> <p>Communication/Fairness- Resolves conflict through respectful dialogue.
