Evaluates patient's previous living circumstances in context of current medical status to develop an appropriate discharge plan. Minimizes patient anxiety concerning illness and facilitates post- hospital referrals. Interacts with and supplements the health care team in order to enhance the social, emotional and psychological care of the patient/family/significant others. Willing and able to travel to multiple worksites.</p> <p>Job Requirements</p> <p>Minimum Education Requirement:</p> <p>MSW is required from an accredited school of Social Work.</p> <p>Minimum Experience Requirement:</p> <p>Minimum of 1 year experience working in a hospital environment is preferred.
