<p>Company Description</p> <p>TCARE is a mission-driven, evidence-based caregiver support organization that leverages a combination of innovative technology and compassionate support to reduce burnout and improve the wellbeing of caregivers across diverse populations including Medicaid, Medicare, LTSS, IDD, child welfare, veterans and workforce populations. With our national footprint and advanced caregiver care management platform, we deliver tailored care plans and compassionate support through a diverse, interdisciplinary team of advocates, specialists, and clinical leaders, dedicated to delivering meaningful impact and measurable outcomes.</p> <p>Role Description</p> <p>As a TCARE Specialist, you will play a critical role in assessing and supporting caregivers throughout their journey. Using the TCARE Family Caregiver Support platform, you will develop and implement personalized care plans designed to reduce burnout, stress, and depression among caregivers. Your role will involve connecting caregivers to community resources, providing ongoing support, and ensuring positive outcomes throughout their experience with TCARE.</p> <p>We are seeking?clinicians with experience in child welfare, foster care, or family services, who understand the unique challenges caregivers face when supporting vulnerable children and families. </p> <p>Key Responsibilities</p> <ul> <li> Become a TCARE Expert: Complete the TCARE Certification course and adhere to best practices to deliver high-quality care management services. </li><li> Conduct Assessments: Administer the TCARE assessment by phone, evaluating caregiver needs and creating tailored care plans to address their unique challenges. </li><li> Develop Care Plans: Design comprehensive care plans that mitigate caregiver stress and burnout, providing clear explanations of the services offered. </li><li> Connect and Support: Link caregivers with relevant social services and community-based programs to enhance their well-being and address identified needs. </li><li> Continuous Improvement: Perform ongoing follow-up assessments and adjust care plans as necessary to ensure continuity of care and positive outcomes. </li><li> Critical Thinking: Apply ethical standards and critical thinking skills to make informed referrals when risk or danger is identified. </li><li> Build Relationships: Utilize motivational interviewing and empathetic listening to foster strong, supportive relationships with caregivers. </li><li> Achieve KPIs: Meet targeted Key Performance Indicators and contribute to case and team meetings as required. </li></ul> <p>Required Qualifications</p> <ul> <li> Educational Background: Bachelor's degree in social work, counseling, psychology, or a related field required. Master's degree (MSW, Counseling, MFT) or clinical license preferred. </li><li> Experience: Minimum of 2 years of?child welfare, foster care, family services, or protective services experience?required. Additional experience in community-based services, public health, or case management strongly preferred. </li><li> Skills: Strong communication skills, both written and verbal, with excellent organizational and time management abilities.
