This position is remote but requires hybrid training on site in Independence, OH.</p> <p>A caregiver who excels in this role will:</p> <ul> <li> <p>Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse.</p> </li><li> <p>Coordinate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services.</p> </li><li> <p>Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate.</p> </li><li> <p>Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process.</p> </li><li> <p>Serve as the CCHCS information specialist.</p> </li><li> <p>Understand other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines.</p> </li></ul> <p>Minimum qualifications for the ideal future caregiver include:</p> <ul> <li> <p>Graduate from an accredited Practical Nursing Program</p> </li><li> <p>Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)</p> </li></ul> <p>Preferred qualifications for the ideal future caregiver include:</p> <ul> <li>One year of clinical experience in Homecare </li></ul> <p>Our caregivers continue to create the best outcomes for our patients across each of our facilities.
