<p>Position Summary</p> <p>The Community Outreach Manager, RN (COM) is a Registered Nurse who leads and supports KCHC's Community Outreach Department by coordinating outreach initiatives, supervising care coordination efforts, providing direct nursing services, and health education services to improve community health outcomes. This position bridges clinical care with community engagement, helping patients overcome social barriers to health by linking them to appropriate services and advocating on their behalf. The COM plays a critical leadership role in aligning outreach operations with KCHC's mission and compliance standards while ensuring culturally competent care.</p> <p>Qualifications</p> <p>Education:</p> <ul> <li>Associate or Bachelor of Science in Nursing (BSN preferred) </li></ul> <p>Certifications & Licenses:</p> <ul> <li>Active and unencumbered North Carolina RN license </li><li>Valid NC driver's license; insurable on company policy </li><li>CPR certification (or obtained within 6 months) </li></ul> <p>Experience:</p> <ul> <li>3+ years of clinical experience required </li><li>2+ years of supervisory or outreach coordination experience strongly preferred </li><li>Bilingual (English/Spanish) preferred </li></ul> <p>Skills:</p> <ul> <li>Knowledge of care coordination, public health, and social determinants of health </li><li>Proficient in Microsoft Office Suite, telehealth tools, and mobile technology </li></ul> <p>Essential Duties and Responsibilities</p> <p>Clinical Services & Health Education</p> <ul> <li>Provide nursing services, health screenings, triage, and chronic disease education using motivational interviewing and coaching techniques. </li><li>Educate patients on conditions such as diabetes, hypertension, and behavioral health-both in person and via telehealth/mobile outreach. </li><li>Support underserved populations, including migrant and seasonal workers, by addressing health risks and promoting self-management. </li></ul> <p>Care Coordination & Patient Navigation</p> <ul> <li>Manage a caseload of up to 100 patients, supporting care coordination and addressing social determinants of health (e.g., housing, food, job training). </li><li>Register patients, verify sliding scale eligibility, and maintain accurate records in the EHR. </li><li>Coordinate referrals, schedule appointments, conduct follow-ups, make home visits, and arrange transportation as needed. </li></ul> <p>Outreach & Community Engagement</p> <ul> <li>Identify individuals needing a primary care home and connect them with services. </li><li>Represent KCHC at community events and meetings; collaborate with partners such as NC Field, Prevent Blindness, and the Migrant Education Program. </li><li>Lead planning and logistics for mobile unit outreach efforts. </li></ul> <p>Program Oversight & Team Support</p> <ul> <li>Supervise daily outreach operations and support team training and development. </li><li>Collaborate with clinical and administrative teams to streamline care delivery. </li><li>Monitor documentation quality and ensure timely EHR entries across the team. </li></ul> <p>Reporting, Documentation & Compliance</p> <ul> <li>Responsible for auditing vehicle safety checks and mileage logs monthly. </li><li>Perform monthly chart audits (minimum 10) and submit required performance reports. </li><li>Ensure compliance with HIPAA, HRSA, and KCHC policies and grant/contract requirements. </li></ul> <p>Professionalism & Cultural Competency</p> <ul> <li>Maintain bilingual communication skills as qualified and provide culturally competent support, including facilitating interpretation coordination as needed. </li><li>Remain current with required trainings and certifications; work independently within scope and policy. </li><li>Maintain company-insured driver eligibility and notify leadership of barriers to role performance. </li></ul> <p>Other Duties</p> <ul> <li>Perform additional tasks as assigned to support organizational goals. </li></ul> <p>Work Environment & Schedule</p> <ul> <li>Must be able and comfortable working in a variety of settings including, but not limited to, clinical environments, office spaces, and outdoor locations. </li><li>This role may involve exposure to low, normal, and high temperatures, as well as varying weather conditions during outreach or event-based activities. </li><li>Must be able to work nights and weekends as departmental needs arise. </li></ul> <p>Travel Requirements</p> <ul> <li>Regular local travel is required using a company vehicle. </li><li>Employee must adhere to all vehicle safety policies and participate in routine vehicle inspections and in-service trainings. </li></ul> <p>Core Competencies</p> <ul> <li>Communication: Demonstrates strong verbal, written, and digital communication skills;
