We will offer the vaccination at no cost via our Health Centers.</p> <p>SERVICE AREA: Housing & Justice Related Services</p> <p>PROGRAM: IPP Case Management</p> <p>JOB OVERVIEW:</p> <p>The full-time Bilingual IPP Case Manager reports to the IPP Case Management Supervisor. This individual works directly with the participants court court-stipulated for treatment services with oversight from the Adult Probation and Parole departments. The primary responsibilities are to ensure that services are provided to participants and documented according to DAP, CBH, Forensic Services, and PHMC requirements. This individual will provide support to the participants while working with all levels of management within the treatment program, Forensic Services, and the recovery community.</p> <p>RESPONSIBILITIES:</p> <ul> <li>Establish and maintain variable service productivity targets depending on the needs of the program. </li><li>Provide case management services to chemically dependent/co-occurring individuals with a caseload of 25 active participants. Primary caseload will consist of a mix of bilingual Spanish-speaking individuals. </li><li>Documentation of service provision within 24 hours of service completion </li><li>Active participation in team meetings, scheduled case conferencing, and trainings as assigned </li><li>Collaboration with identified court personnel, probation/parole, drug and alcohol treatment providers, and other assigned agencies as identified </li><li>Assist individuals to link to recovery support services, including Pro-ACT recovery center, Access to Recovery, faith-based institutions, mental health services, and other identified community supports. </li><li>Ensure weekly work schedules are updated with detailed activities utilizing the program's Outlook feature. </li><li>Maintain participant records in accordance with agency fee-for-service standards, managed care, and licensing requirements. </li><li>Prioritize assisting participants to enroll in drug and alcohol treatment, physical health services, DPA, vocational services, and educational services. </li><li>Identify, coordinate, and link community resources to the participant to promote accessibility to recovery-oriented community services. </li><li>Coordinate and communicate with other programs within and outside the agency to foster a collaborative team approach. </li><li>Average of 95% work week in the community for participant appointments and provider meetings </li><li>Consideration for flexible work schedules available if needed to provide participant services during non-traditional hours, based on participant and departmental needs </li></ul> <p>SKILLS:</p> <ul> <li>Valid driver's license required </li><li>Good customer service skills </li><li>Ability to work remotely in the community, accessing public transportation </li><li>Good time management skills </li><li>Excellent oral/written communications skills with the ability to type </li><li>Good analytical and problem-solving skills </li><li>Must be able to speak fluently in Spanish </li></ul> <p>QUALIFICATIONS:</p> <ul> <li> <p>Must meet one of the following:</p> </li><li> <p>Associate degree in social services, sociology, psychology, criminal justice, or social work and 2 years of experience in public and private human services including 1 year in direct participant contact in an addiction recovery, mental health, or behavioral health setting</p> </li><li> <p>Bachelor's degree with major course work in sociology, social work, psychology, anthropology, political science, history, criminal justice, counseling, or education and at least 1 year of direct participant contact in an addiction recovery, mental health, or behavioral health setting</p> </li><li> <p>Preferred: Experience in direct case management services with a chemically dependent criminal justice population</p> </li><li> <p>Preferred: Experience in fee-for-service delivery</p> </li></ul> <p>PAY GRADE: 16</p> <p>PHMC is an Equal Opportunity and E-Verify Employer.</p>
