<p>Job Overview</p> <p>This position responsible and accountable for care coordination and medical co-management for patient enrolled in the Complex Care Program. Serves as the accountable nursing leader in the development of the complex care program. Together with the team of clinical staff, prioritizes and delivers patient care within regulations in accordance to their licensure within the Commonwealth of MA.</p> <p>Hours: Full- Time: Days</p> <p>Location: Lowell General, Main Campus</p> <p>Job Description</p> <p>Minimum Qualifications:</p> <ol> <li> <p>Bachelor of Science in Nursing (BSN).</p> </li><li> <p>Registered Nurse (RN) license.</p> </li><li> <p>Basic Life Support (BLS) or Pediatric Advanced Life Support (PALS) certification</p> </li><li> <p>Eligible for state and federal controlled substances registration.</p> </li><li> <p>Extensive clinical experience including project management, program development, and supervision.</p> </li></ol> <p>Preferred Qualifications:</p> <ol> <li> <p>Masters Degree</p> </li><li> <p>Advanced Practice Nursing (APRN) license.</p> </li><li> <p>Bilingual</p> </li></ol> <p>Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.</p> <ul> <li>Provides direction and sets nursing care priorities for patient care within the complex care program. </li><li>Demonstrates commitment and expertise in family centered care. </li><li>Contributes to the development of effective care interventions and tools for the Complex Care Program. </li><li>Establishes priorities for each patient/family in collaboration with the interdisciplinary care team. </li><li>Facilitates care provided by individuals and departments outside of the Floating Hospital, seeking appropriate support to alleviate identified obstacles to efficient care. </li><li>Effectively performs role based on established metrics of care efficiency. </li><li>Demonstrates ability to create and maximize teamwork. </li><li>Demonstrates clinical ability as an advance practice provider. </li><li>Leads by establishing quality assessments and improvement activities. </li><li>Performs within the role functions of NP and/or PA when indicated, and within job descriptions. </li><li>Develops and maintains affinity between all members of the complex care administrative and Clinical staff, thus producing high performing, patient focused teamwork. </li><li>Develops and maintains affinity between all supporting departments, clinical areas, and staff outside the Floating Hospital, thus producing high performing, patient focused teamwork. </li><li>Provides formal feedback to all members of the care team and support department. </li><li>Provides input to a budget and provides oversight to the nursing aspects of resource allocation and development of the program. </li><li>Sets goals and reports on outcomes to designated hospital committees. </li><li>Identifies and works with hospital leaders and members of the care team to remove barriers in development and operations of the program. </li></ul> <p>Tufts Medicine is a leading integrated health system bringing together the best of academic and community healthcare to deliver exceptional, connected and accessible care experiences to consumers across Massachusetts.
