<p>JOB SUMMARY:</p> <ul> <li>Responsible for the coordination of clinical activities of the Wound Care Center. </li><li>These include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through the use of hospital and nursing standards. </li></ul> <p>JOB SPECIFIC CORE COMPETENCIES:</p> <ul> <li>Ability to problem solve using strategic critical thinking skills </li><li>Must show attention to detail in all aspects of your daily work </li><li>Must demonstrate effective communication skills within all levels of communication while performing your job </li></ul> <p>ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:</p> <ul> <li>Provides case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patient care. </li><li>Provides efficient and effective care with the guidance of the physician during the clinic sessions, attends the clinics where scheduled and is dependable. </li><li>Able to manage a number of patients at the same time with high energy and organization. </li><li>Collaborates with other health care providers, wound care center physicians, Program Director, Medical Director and Supervisor regarding clinic and patient needs. </li></ul> <p>EXPECTED BEHAVIORS:</p> <ul> <li>Bases care on the nursing process. </li><li>Demonstrates thorough, accurate and timely assessments while exhibiting necessary response to changes in the patient's condition. </li><li>Provides and documents care per hospital and department policies and procedures. Demonstrates competence in clinical and technical skills. </li><li>Provides instructions to patient/families regarding cardiology procedures and care following discharge. </li><li>Seeks educational opportunities to improve knowledge/skill. </li><li>Participates in identifying or recommending areas for improvement. </li><li>Completes work in an efficient, timely manner that is conducive to team work and cost containment. </li><li>Communicates in a clear, concise and professional manner. </li><li>Performs nursing care following safety principles, OSHA guidelines and infection control guidelines. </li><li>Provides patient care following the Patient's First Commitment, AIDET, and customer service training. </li></ul> <p>CERTIFICATION/LICENSE: Cardio-Pulmonary Resuscitation (CPR); Registered Nurse (RN).</p> <p>ADDITIONAL LICENSURE/CREDENTIAL REQUIREMENTS: Current licensure by the state board of nursing RN. ASN or BSN from an accredited nursing program.</p> <p>ADDITIONAL EDUCATION AND EXPERIENCE REQUIREMENTS: Mandatory Annual Continuing Education: Customer Service, Fire and Safety, Confidentiality-HIPAA, Infection Control, Corporate Compliance. Annual Unit Specific Competencies, and all education required by regulatory, accreditation bodies and/or Hancock Regional Hospital.</p> <p>PHYSICAL/MENTAL DEMANDS:</p> <ul> <li>Lift to 50 lbs. independently and for weights greater than 50 lbs. must use mechanical device or team lifting. </li><li>Push/Pull patients of up to 250 lbs. in a wheeled device and for weights greater than 250 lbs. must use team assistance. </li><li>Install equipment weighing up to 10 lbs. (trapeze, traction) overhead in patient rooms </li><li>Manual dexterity, as used for performing intricate procedures (catheter, dressing changes, IV starts), assembling medical equipment, caring for patients, typing, etc. </li><li>Must be able to lift from ground level up above own head </li><li>Sitting up to one hour of day for computer documentation, written reports </li><li>Squatting and bending at waist to reach under beds, repetitive crouching as needed </li><li>Standing, walking, stooping, kneeling, crouching and crawling as needed more than 2/3 of day </li></ul> <p>EQUIPMENT USED: Hyperbaric Oxygen Chambers, Pyxis, Sensilase, TCOM.</p> <p>ENVIRONMENTAL CONDITIONS: Long periods of standing and walking.</p>
