<p>Company:</p> <p>Mercy Health by Compassus</p> <ul> <li>THIS IS A COLLECTIVE BARGAINING UNIT POSITION* </li></ul> <p>Position Summary</p> <p>The Home Health Aide (Union T-SVC) is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Aide (Union T-SVC) provides basic care and supports the plan of care developed for home care patients under direct supervision.</p> <p>Position Specific Responsibilities</p> <ul> <li>Participates in orientation of newly hired HHA's </li><li>Participates in the quality improvement process. </li><li>Provides services as outlined in patient care plan. Ensures patient's overall appearance reflects good hygiene, patient's environment is neat and orderly. Reports observations of patient condition to RN in a timely manner. </li><li>Promptly responds to patient requests and interacts with patients, families and other care givers in an emotionally supportive way. </li><li>Completes and submits documentation according to policy. </li><li>Demonstrates compliance with federal, state and local rules and regulations, along with the HMHP Corporate Responsibility Program. </li><li>Participates in at least 75% of home care monthly staff meetings. </li><li>Participates in appropriate committee/task force meetings. </li><li>Demonstrates skill in conflict resolution. </li><li>Maintains availability at all times during work hours. </li><li>Maintains confidentiality of patient information. </li><li>Demonstrates appropriate and cost effective use of supplies and equipment. </li><li>Assumes on-call responsibility as required by agency. </li><li>Performs other duties as assigned. </li></ul> <p>Education and/or Experience</p> <ul> <li>High School Diploma or GED preferred. </li><li>Completion of training course for Home Health Aide. </li><li>Orientation as Home Health Aide </li></ul> <p>Skills</p> <ul> <li>Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. </li><li>Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications. </li><li>Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy. </li><li>Other Skills and Abilities: Assists the registered nurse and practical nurse by giving patient care in simple nursing situations; must accurately document procedures performed; must accurately complete assignments and follow directions; must report any unusual observations and patient care needs to the Home Care Nurse. </li></ul> <p>Certifications, Licenses, and Registrations</p> <ul> <li>Home Health Aide certification </li><li>CPR certification </li></ul> <p>Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
