<ul> <li>Assists patients and families to adjust to illness and lifestyle changesrelated to diagnosis, illness and treatment. </li><li>Participates in screening for Distress through appropriate tools and provides timely interventions to address psychosocial distress. </li><li>Completes psychosocial assessment and works with the patient and family members to develop a plan for intervention based on mutually agreed upon goals to maximize functioning through treatment and recovery. </li><li>Provide for or arrange for individual, family and group counseling and other appropriate interventions to alleviate distress and enhance functioning. </li><li>Identifies and utilizes resources to assist patient to adjust to changes in function, maximize independence and functioning. </li><li>Assists in patient care planning to ensure patient has resources to meet the psychosocial, emotional and practical needs of a cancer illness. </li><li>Evaluates the ongoing medical and psychosocial needs for home care </li><li>planning in order to set up appropriate community services. </li><li>Evaluates the current treatment plan, available social service programs and formulates a pertinent psychosocial care plan that will provide maximum benefit for each patient. </li><li>Assists patient/family in the procurement of essential equipment to allow </li><li>optimum and safe function at home. </li><li>Assists patient/family in the decision making process leading to the </li><li>selection of and placement in an appropriate long term care facility if appropriate. </li><li>Facilitates discussion of end of life issues and provides resources and education to patient and family to allow for informed decision making and reduced distress\ </li><li>Provides information and referral for continuing care and crisis </li><li>intervention. </li><li>Provides education to the patient, family and team regarding specialized cancer related community resources and programming related to the individual needs of the patient. </li><li>Refers voluntary community based services to management such as community food pantries, charitable financial support organizations, as appropriate to enhance coping and provide for immediate patient and family needs. </li><li>Functions as advocate for patients as they interact with appropriate community agencies. </li><li>Evaluates the appropriateness of community services as possible and develop integrated plans to meet patient needs. </li><li>Works cooperatively with co-workers and communicates effectively with </li><li>departmental staff; other departmental personnel and health care </li><li>facilities; physicians, patients and visitors. </li><li>Acts in accordance with departmental policy and procedure. </li><li>Functions within the professional National Association of Social Workers (NASW) Code of Ethics. Follows the Associate of Oncology Social Work Standards of Practice. </li><li>Is available to all clinical areas in the Oncology Department for Social Work services as needed and assigned. </li><li>Documents all OSW interactions with team, patient and family in the Electronic Medical Record for appropriate team communication, collaboration and continuity of care. </li><li>Maintains monthly statistical data. </li><li>Observes all safety, infection and environmental control procedures. </li><li>Functions in such a way as to meet departmental quality assurance and process improvement programming. </li><li>Attends and actively participates in meetings and serves on committees as </li><li>requested. </li><li>As assigned, participates in Tumor Conferences, Cancer Committee meetings and other Oncology related on-site meetings. </li><li>Performs other work related duties as requested/directed. </li></ul> <p>Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects Cape Cod Hospital's commitment to CARES: compassion, accountability, respect, excellence and service.</p> <ul> <li>Master's degree in Social Work (MSW) and State licensure (L.I.C.S.W.) </li><li>Minimum of two years Clinical Social Work in a health care setting </li><li>Experience with Oncology patients preferred </li><li>OSW0C Credential preferred </li></ul>
