<p>Current Employees: If you are currently employed at SOMC please log into UKG Pro to use the internal application process.</p> <p>Department: Homecare</p> <p>Shift/Schedule: Contingent (Works as needed)</p> <p>GENERAL SUMMARY</p> <p>Works under the supervision of the Nurse Manager or Asst Nurse Manager. The Home Care Aide's primary job function is to: According to established procedures, performs routine service duties and basic nursing procedures necessary to provide for care, comfort and safety of Home Care patients. In doing so, takes vital signs, assists patients with personal hygiene and feeds and ambulates patients according to specific procedures. May help maintain patient's immediate area and may occasionally run errands for Home Care patients. Performs other duties as assigned.</p> <p>QUALIFICATIONS</p> <p>Education:</p> <ul> <li>High School Diploma or successful completion of an equivalent High School Exam Required </li><li>Successful completion of Home Care competency evaluation program required </li></ul> <p>Licensure:</p> <ul> <li>BLS within 90 days of hire required </li><li>BMVL required (Must maintain eligibility to drive as determined by SOMC insurance carriers) </li><li>Completion of approved nursing assistant, home care aide, or long term care nursing assistant training preferred </li></ul> <p>Experience:</p> <ul> <li>Approximately six months prior experience in either the acute, long term care or community based care setting preferred. Home Care experience preferred. </li></ul> <p>JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS</p> <p>The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.</p> <ol> <li> <p>Makes home visits and provides for care, comfort and safety of assigned patients under direction of the Home Care registered nurse.</p> </li><li> <p>Performs personal care activities.</p> </li><li> <p>Performs selected basic patient care procedures such as enemas, decubitus care, non-sterile dressings, and vital signs.</p> </li><li> <p>Assists patient with transfers, ambulation and performs R.O.M. exercises according to the Plan of Care.</p> </li><li> <p>Performs light household services such as: linen changes, light housekeeping in the patient areas, laundry, meal preparation and running errands.</p> </li><li> <p>Observes and reports environmental, physical and emotional changes in the patients' status to the Home Care Nurse.</p> </li><li> <p>Documents care and services provided and observations reported to the nurse.</p> </li></ol> <p>Thank you for your interest in Southern Ohio Medical Center.
